Planned Parenthood of Orange and San Bernardino Counties has a full-time opportunity for a Centralized RN Case Management Coordinator in City, CA.
Provide centralized case management including follow up care in accordance with Planned Parenthood regulations. Monitor follow up of abnormal labs, diagnostic studies, laboratory test results, and external care referrals in accordance with Planned Parenthood regulations and protocols and Board of Registered Nursing. Assist in management and follow-up of high-risk patient cases as requested by the Medical Director. Provide back-up and support for case management functions that must be performed by a licensed nurse, including follow up of all positive lab results assigned to the case management department. Participate in activities related to incident reporting including external reporting to ARMS. Provide support to the clinical staff by receiving calls from patients with clinical concerns, when clinician is not immediately available.

Responsibilities
Essential Functions: Essential functions encompass the required tasks, duties and responsibilities performed as part of the job and the reason the job exists.
Provides standardization of centralized patient case management of abnormal labs, telephone encounters and follow-up for all affiliate centers according to PPFA and PPOSBC’s Medical Standards and Guidelines. Ensure compliance with PPFA protocols for follow-up.
Report incidents through the AIMS reporting system as assigned. Update log of incident reports as entered.
Review Melody schedule to identify patients that did not show for their appointment and initiate follow up if needed.
Answer health related questions from patients during business hours.
Schedule appointments and screen as needed for family planning/primary care including but not limited to contraceptives and abortion services as needed.
Provide medication abortion follow up.
Assist in management of high-risk cases including expediting the referral process for
high-risk cases, assisting the patient in identifying funding for their care, and reviewing and scanning records of evaluations done outside the affiliate.
Cross train in additional case management tasks and provide coverage and assistance to other members of the case management team as needed.
Working collaboratively with Department of Public Health with linkage of care and services for patients with abnormal reportable labs.
Assigned as Centralized RN to assist health centers with tasks such as hospital transfers and following up with patients.
Maintain electronic logs related to Incident Reporting, triage calls, and follow-up.
Attend meetings at both on and off-site locations as needed.
Other duties as assigned by Supervisor/Director of Case Management
Required to take after hours call, as needed.
Collaborate with Clinicians/Providers as needed to ensure quality care regarding patient follow-up including but not limited to scheduling appointments, coordinating outside referrals and outcomes.
Keep Clinician/Supervisor apprised of any problems, incidents or potential problems involving patients or staff as relates to any services.

Licensure and/or Certification Requirements:
Valid California Registered Nurse’s License
CPR Certification
Minimum Education:
Associate’s Degree in Nursing
Minimum Work Experience:
One or more years of clinical experience, including case management preferred.
Language Requirements/Preference:
Bilingual in Spanish is preferred.
