In collaboration with Director of Integrated Care Management, manages the operations of the Care Management Department to include Care Management, Discharge and Transition Planning and Utilization Management in accordance with regulatory requirements and the needs of the hospital. Assumes shared responsibility for ensuring optimal clinical and financial outcomes across provider lines and systems in conjunction with all members of the healthcare team.

Qualifications
Education and Experience
One of the following required: Bachelor of Science in Nursing or Master of Social Work.
One of the following required: Current RN license in State of Maryland or current Maryland Licensed Clinical Social Worker (LCSW).
Minimum five (5) years of previous case management experience required which must include at least two (2 years of previous supervisory experience.
