Our client is seeking a dedicated and licensed Assistant Director of Social Services for our skilled nursing facility in Newton, MA. The ideal candidate must be licensed in the state of Massachusetts and have experience working in a Skilled Nursing Facility (SNF).
Position Summary:
The Assistant Director of Social Services will help plan, organize, implement, evaluate, and direct the Social Services Department's operations. This role involves administrative authority, responsibility, and accountability to ensure quality care and services for our residents.
Key Responsibilities:
Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs of the facility.
Participate in discharge planning, resident assessments, and the development of individualized social care plans.
Conduct interviews with residents and families to gather social history information.
Refer residents and families to appropriate community agencies and resources when needed.
Provide consultation and guidance to staff and community agencies to support residents' social and emotional needs.
Communicate residents' social, psychological, and emotional needs to the care team.
Help secure resources from community agencies to meet the needs of residents.
Ensure the confidentiality and protection of residents' health information.
Administrative Duties:
Assist in the development and administration of departmental policies and procedures.
Review and recommend updates to policies and procedures annually.
Complete required documentation, forms, and reports accurately and promptly.
Coordinate transportation arrangements for residents when necessary.
Provide residents and families with information about available financial assistance programs.
Assist in the annual review and updating of job descriptions within the department.
Delegate tasks and provide recommendations to the Director of Social Services.
Develop standardized methods to address the social and practical needs of residents.
Qualifications:
Bachelor's degree in Social Work (BSW) or a related field.
At least one year of supervised experience in a healthcare setting.
Current Social Work licensure in the state of Massachusetts.
Experience working in a Skilled Nursing Facility (SNF) is required.
