GENERAL DESCRIPTION:

Under the general supervision of the RN or LPN Unit Manager, the Licensed Practical Nurse provides nursing care to residents of the nursing home.

QUALIFICATIONS: An example of acceptable qualifications:

Completion of secondary education or equivalent (high school diploma or GED); completion of a state accredited school of licensed practical nursing; completion of state approved pharmacology program; related experience in a nursing facility; or any equivalent combination of education, experience, and/or training which provides the required knowledge, skills, and abilities. An individual who poses a direct threat to the health and safety of himself/herself or others in the workplace will be deemed not qualified for this position.

LICENSURE OR CERTIFICATION REQUIREMENTS:

Must possess and maintain an Ohio Practical Nurse License issued by the Board of Nursing; IV certification.

EQUIPMENT OPERATED: The following are examples only and are not intended to be all inclusive:

Computer, computer software (e.g., Microsoft Office, Outlook, Point Click Care, and other applicable computer software), printer, copy machine, calculator, fax machine, other standard business office equipment, medical equipment.

INHERENTLY HAZARDOUS OR PHYSICALLY DEMANDING WORKING CONDITIONS:

Employee has exposure to chemical compounds found in an office environment (e.g., toner, correction fluid, etc.); may have contact with potentially violent or emotionally distraught persons; may have contact with blood, bodily tissues, and fluids; works second and third shift; this is considered medium work and the employee must demonstrate the ability to perform the physical demands required of the position in accordance with the U.S. Department of Labor’s physical demands strength ratings.

In cases of emergency, unpredictable situations, and/or department needs, may be required to lift, push, pull, and/or carry objects heavier than D.O.L. strength ratings recommend.

JOB DESCRIPTION AND WORKER CHARACTERISTICS:

JOB DUTIES in order of importance

ESSENTIAL FUNCTIONS OF THE POSITION: For purposes of 42 USC 12101:

60% (1) Delivers nursing care in accordance with physicians’ orders and with acceptable nursing standards and practices in accordance with federal, state, and local standards (e.g., passes medication; charts medication administered; verifies that medication corresponds with physician orders; ensures med-carts are locked when not in use and counts narcotics at the end of each shift; obtains and records residents’ vital signs; makes regular rounds to monitor health conditions of residents and records and reports changes in condition to the Unit Manager or RN Clinical Supervisor; responds to emergency situations; starts IV’s and completes IV flow sheets; inserts, irrigates, and removes catheters; checks temperature, pulse, respiration, and blood pressure; administers enemas; administers oxygen; performs tracheotomy cleaning and suctioning; changes dressings; assists with feeding residents and tube feeds residents, etc.).

28% (2) Receives and gives shift reports to the Unit Manager; completes all necessary documentation in nurse’s notes, care plans, and medication administration records; assists with preparation of transfers and resident transports; arranges for x-rays and stat laboratory work; prepares and maintains records, reports, and charts; prepares Tx and Rx sheets.

5% (3) Monitors the work and care delivered by STNA’s and resident services aides; provides other resident care as needed (bathing, toileting, etc.).

2% (4) May assist the dining room serving meals, as needed.

(5) Attends meetings and serves on committees, as directed; attends training and seminars, as directed.

(6) Maintains required licensure or certification, if any.

(7) Meets all job safety requirements and all applicable safety standards that pertain to essential functions.

(8) Demonstrates regular and predictable attendance.

OTHER DUTIES AND RESPONSIBILITIES:

5% (1) Performs other related duties as necessary.

(2) Required to work second and third shift.

MINIMUM ACCEPTABLE CHARACTERISTICS: (*indicates developed after employment)

Pay: $27.06 – $29.18 per hour

 

Housekeeping/Laundry Worker

Full - Time

7:00 AM - 3:00 PM

GENERAL DESCRIPTION:

Under immediate supervision of the Custodial Work Coordinator, the Utility Worker performs housekeeping and laundry duties; works in housekeeping and laundry departments as workload and staffing needs dictate.

QUALIFICATIONS: An example of acceptable qualifications:

Completion of secondary education or equivalent (high school diploma or GED) or any equivalent combination of education, experience, and/or training which provides the required knowledge, skills, and abilities. An individual who poses a direct threat to the health and safety of himself/herself or others in the workplace will be deemed not qualified for this position.

LICENSURE OR CERTIFICATION REQUIREMENTS:

None.

EQUIPMENT OPERATED: The following are examples only and are not intended to be all inclusive:

Standard custodial equipment (mop, broom, buffer, scrubber, washer, dryer, etc.).

INHERENTLY HAZARDOUS OR PHYSICALLY DEMANDING WORKING CONDITIONS:

Employee is exposed to environmental conditions which may result in injury from fumes, odors, dusts, mists, gases, and/or poorly ventilated work areas; is exposed to possible injury as a result of working with moving mechanical parts of equipment or machines; this is considered light work and the employee must demonstrate the ability to perform the physical demands required of the position in accordance with the U.S. Department of Labor’s physical demands strength ratings.

In cases of emergency, unpredictable situations, and/or department needs, may be required to lift, push, pull, and/or carry objects heavier than D.O.L. strength ratings recommend.

45% (2) Cleans and sanitizes resident rooms; cleans and sanitizes bathrooms including toilets and sinks; cleans and sanitizes all surfaces including furniture, windows, walls, and woodwork; cleans nurses station; collects and takes trash to the dumpster; performs basic floor care; sweeps, mops, buffs tile, extraction cleans and buffs carpet; changes cubicle curtains and drapes, as requested; waters plants; ensures the security of cleaning supplies; reports broken equipment and unsafe conditions to the supervisor; records completed work on work charts; performs additional custodial tasks, as required.

(3) Attends meetings and serves on committees, as directed; attends training and seminars, as directed.

(4) Maintains required licensure or certification, if any.

(5) Meets all job safety requirements and all applicable safety standards that pertain to essential functions.

(6) Demonstrates regular and predictable attendance.
