Compensation: $25.54/hr
The PATH coordinator locates and connects consumers who are facing housing challenges with programs and services to facilitate positive outcomes and housing stability.
ESSENTIAL FUNCTIONS - JOB SPECIFIC:
* Locates, facilitates, educates, coordinates and leverages available housing programs and resources to assist consumers and clinical support teams in navigating, securing and sustaining positive housing outcomes
* Practices Housing First and other low barrier service delivery models
* Attends local Continuum of Care meetings related to coordinated entry/case conferencing
* Receives referrals from Continuum of Care programs to assist individuals with housing stability
* Receives and processes internal referrals from ERE, CPRC SUD, etc.
* Outreach to individuals not currently engaged in services who are in a housing crisis or experiencing homelessness
* Refers and transports individuals, as needed, to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits
* Recruits landlords and build rapport by directly contacting potential landlords to provide information and make referrals to their units. Follow up with landlords about referrals and problem-
* Obtains SOAR certification and assist individuals throughout the SOAR training process
* Assists with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options
* Documents all grant activities in HMIS
* Participates in local community events related to housing resources.
* Provides training, coaching and supporting in daily living skills, including housekeeping, cooking, personal grooming, accessing transportation, keeping a budget, paying bills and maintaining an independent residence
* Other duties as assigned
* High School/GED required
* Bachelor's degree in social work, sociology, psychology or related field preferred
* At least four (4) years' experience or a combination of education/experience in human services required if Bachelor's degree is not obtained
* The ability to obtain and maintain a Class E MO driver's license by the 60-day introductory period required
