Job Summary:

The Kentucky Moms MAT-R Case Manager holds the primary responsibility for evaluating individuals’ needs and coordinating care and services as needed/requested. They also provide consultation and training to other partner agencies as well as work in conjunction with the Kentucky Moms MAT-R Prevention Specialist. Participation in workshops, trainings, resource fairs, healthy baby workshops, etc. is expected. This position works collaboratively with the Regional Prevention Center (RPC).

Qualifications:

Education and Experience:

• Proven ability to build relationships, trust and deepen connections with diverse people and groups.
• Bachelor’s degree, preferably with emphasis in Behavioral Science/Human Services, with 1-year relevant experience.
• Must maintain a valid driver’s license with acceptable driving record throughout employment as driving is required; use of personal vehicle and maintenance of personal car insurance is required.
