Acute Long-Term Care Hospital

An exciting opportunity in a professional hospital setting to assist patients transition from short-term to post-acute care venues, including inpatient rehabilitation, skilled nursing, and home care. Grace Hospital is a not-for-profit registered Acute Long-Term Care Hospital, the first of its kind to be established in Cuyahoga County. We are Medicare and Medicaid certified and fully accredited by the Joint Commission. Grace Hospital utilizes an interdisciplinary care team model to produce optimal clinical outcomes. Job Summary: Performs tasks assigned by the Charge Nurse or Registered Nurse.

Qualifications: Must have current CPR!

Education: High school graduate or equivalent. STNA preferred

Experience: 1-3 years of Hospital experience or Nursing Home experience preferred

Other: Ability to accurately perform assigned patient care duties under the direction of the RN; effective

communication skills; able to adapt to new situations. Basic Life Support Certification.

Responsibilities:

• Participates in the development of patient care plans, individualized to and appropriate for the patients' needs, per Hospital protocol.


• Performs patient care as detailed in the individual's care plan.


• Participates in age-appropriate patient education and training as per Hospital protocol.


• Participates in the pain assessment process, as per Hospital protocol.


• Is aware of and promotes alternatives to restraint usage and follows Hospital protocol in the usage and monitoring of patient restraints.
