CLASSIFCATION SUMMARY:

Volunteers of America Southwest's Cambridge Gardens is an independent living community for seniors 62 and older.  Maintaining independence is very important. We provide housing for seniors who want to live independent lifestyles where they can thrive and age in place. Our senior residents deserve the quality of life that they desire and our commitment to service-enriched housing offers them just that. Under the supervision of the Community Administrator, the Services Coordinator is responsible for case management, community liaison, and assessment/referral services. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:


  • Provides general care management and referral services to all residents needing such assistance.

  • Establish linkages with all agencies and service providers in the community; shops around to determine/develop the best values in service pricing to assure individualized, flexible and creative services for the involved residents.

  • Assembles a director of community services and providers and makes available to residents, family and management.

  • Refers and links the residents of the project to service provider in the general community.

  • Helps residents build informal support networks with other residents, family and friends.

  • Pursues avenues for additional services through private, local, state and federal sources.

  • Establishes and assists in development of new services as necessary for utilization in a service coordination capacity.

  • Educate residents regarding service availability, application procedures, client rights, etc., providing advocacy as appropriate.

  • Maintains files, completes reports and documents contact with residents.

  • Other duties as assigned by the Community Administrator or Director of Housing Services.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:


  • Bachelor's degree in social services or related field and 3-5 years of experience. Prefer experience working with elder population.

  • Working knowledge of community services available to the elderly.

  • Experience with case management.

  • Ability to work with community agencies and advisory members.

  • Proficiency with Microsoft Office suite of products.
