Community Housing Case Manager Team Lead

YMCA of Central Ohio

Rate: $48,000 to $60,000 based on experience

Full Time/Salaried - 40 hours/week

Duties Include:
Provide leadership to a peer case management team.Work with individuals experiencing homelessness to obtain/maintain housing.Utilize Housing First principles to assist clients.Maintain limited case load.Assist case managers with difficult clients.Conduct regular case conferences with clients and service providers as required.Assess clients legal, financial, social, medical, psychological, and other needs related to their housing situation.Develop familiarity with unique challenges and resources as it relates to homelessness.Develop effective, trusting relationships with clients.
