Patient Services
Position Summary:
The Patient Services role is essential to provide Access Community Health Centers (Access) patients with comprehensive clinical care and ensure better health outcomes. This position collaborates with other clinical staff to assess patient needs and coordinate services. The main function of the Patient Services role is to provide case management to at-risk patients and/or prenatal patients and coordinate necessary follow-up care. This position is also responsible for tracking and reporting activities related to case management and referrals.
Population Health: Helps patients with resources for complex social issues, assists with referrals to home health and provides outreach efforts around clinical measures directed by clinical leadership to improve screening and vaccination efforts.
Desired candidate would have the following qualifications:
Required Qualifications:
A minimum of a high school diploma or equivalent and health care experience and/or social services experience or associate degree in health care, social services, or related field
Basic computer skills to be able to send and receive email and use programs such as Microsoft Excel, Word, and Access
Experience in case management
Experience in health benefits counseling
Experience in women’s health issues and/or working with a prenatal population
Experience with diverse cultures
Fluent in Spanish and English
Desired Qualifications:
Previous experience using Epic medical software
Compensation and Benefits:
Our total compensation package includes competitive wages, plus a full range of attractive benefits for regular full and part time staff working a minimum of 20 hours per week:
Compensation starts at $23.75 and is adjusted according to experience.
