Job Description:

BASIC FUNCTION AND RESPONSIBILITY


Provide patient care in the Pulmonary Respiratory Specialty & Comprehensive Care Unit (a critical care step down unit), Pulmonary Procedural Services, Adult Cystic Fibrosis Services as well as outpatient follow up of these patients. Provide overall medical care and management for patients, exercising autonomy in medical decision-making and services provided. Function independently to plan, deliver, and evaluate the health care required by the patient. Focus on caring for hospitalized patients in order maximize resource utilization and to control costs to patients and the institution while providing personalized care. As the patients return to the outpatient setting, assist in providing care and developing relationship with outpatient care providers. In addition to the ability to exercise autonomy and function independently, the individual needs to be able to work under the direction of the attending physician.
CHARACTERISTIC DUTIES AND RESPONSIBILITIES

  • Practice in Pulmonary Medicine and potentially Critical Care units by providing overall medical management and serving as the primary patient manager with responsibility for admissions; supervising plans of care; ordering treatments, tests, and medications; and developing discharge plans with the goal of minimal needed the length of stay. Promote evidence-based care guidelines for the care of critically ill patients, including sedation management, thrombosis prophylaxis, central line management and ventilator management. Assess patient for change in status and institute appropriate interventions including necessary emergency interventions to stabilize patient.
  • Perform and document initial evaluation of newly admitted patients, including complete history and physical examination and, complete admission orders under the direction of the attending physician. Direct patient evaluations and treatment plans in collaboration with attending physicians from the Medical Intensive Care Unit (MICU), Respiratory Specialty Comprehensive Care Unit (RSCCU), Interventional Bronchoscopy Staff, and Adult Cystic Fibrosis Staff.
  • Provide overall medical care, functioning independently to assess, perform medical diagnostic and therapeutic procedures or diagnostic procedures based upon patient's clinical status and response to procedures and completion of patient care orders. Perform or assist with approved therapeutic or diagnostic procedures based upon patient's clinical status. Document patient preparation and response to procedures(s). Participate in daily inpatient rounds with the attending physician. Complete written medical record documentation, including patient progress notes and hospital discharge summary. Coordinate the discharge planning process and appropriate post-discharge follow-up care. Provide assistance in the care of outpatients.
  • Direct and mentor and educate fellows, residents, medical students, nurses, physician assistants, and advanced registered nurse practitioners as to the needs and issues of inpatient population management and optimal resource utilization with minimally needed length of stay.
  • Coordinate and participate in clinical research regarding such topics as cystic fibrosis, critical care physiology, ventilator management, sepsis, acute lung injury, and managing patient length of stay while providing cost effective and high-quality patient care or evaluating the utilization of resources.
  • Order and interpret appropriate diagnostic and laboratory tests. Explain the necessity, preparation, nature and anticipated effects of procedures to patients, patient's family, staff and healthcare learners.
  • Provide anticipatory guidance and counseling to patients and family members including goals of care.
  • Facilitate communication with internal and external health care providers in coordination with the attending physician.
  • Initiate and coordinate patient referrals to appropriate specialty and subspecialty physicians or other health care providers as indicated by the patient's health conditions/diagnosis.
  • Facilitate the appropriate referral of patient to other health facilities, agencies and resources as appropriate.
  • Make sign out rounds with night call and weekend call coverage. Perform essential functions on weekends and Holidays on a rotating basis (1:4), with commensurate relief of weekday functions.
  • Participate in the development, utilization and evaluation of resources to meet patient and family education needs.
  • Provide education and serve as a resource for CCOM and UI Health Care staff and providers.
  • Assure clinical competency and expertise in area of hospital specialization.
  • Maintain an effective working relationship with faculty, staff, trainees and the public.
  • Follow policies and procedures in accordance with the regulations of the University of Iowa Health Care, the Carver College of Medicine, and the Department of Internal Medicine.
  • Perform other duties as assigned within the professional scope of practice.
  • Establish and maintain standards of collaborative interaction among peers and employees that is characterized by respect, honesty and service; assures that all unit members are held to similar standards and ethics.

SUPERVISION RECEIVED


Direct patient care supervision is received from the attending physicians and additional supervision by the Supervisory Hospitalist ARNP. The attending physicians would include but not limited to the Medical Intensive Care Unit (MICU) staff, Respiratory Specialty Comprehensive Care Unit (RSCCU) staff, interventional bronchoscopy staff, and Adult Cystic Fibrosis staff,.
SUPERVISION EXERCISED


Functional supervision may be exercised over support staff.


QUALIFICATIONS


  • Physician's Assistant - Must comply with registration and licensure requirements as specified by the Iowa Board of Physician Assistant Examiners which include graduation from an accredited PA educational program and/or certification by the NCCPA.
  • Advanced Registered Nurse Practitioner (ARNP) - A Masters Degree is required. A Master's degree in Nursing is preferred. If Master's degree is in a related field, a Baccalaureate degree in nursing is required.
  • ARNP - Current certification in an adult specialty area and licensure as an RN and ARNP in state of Iowa is required.
  • Excellent written and verbal communication skills are required.
  • Reasonable (1-3 years) experience in Acute Care or Intensive Care clinical practice is required or previous experience in this specialty area or demonstrated knowledge of this clinical practice is required.
  • Demonstrated knowledge of effective strategies for working with faculty, staff, and students from a variety of backgrounds and perspectives.
  • Experience providing overall medical care and medical management for patients in Acute Care or Intensive Care setting is highly desirable.
  • Experience exercising autonomy in medical decision-making and services provided is highly desirable.
  • Experience managing length of stay, improve resource utilization and decrease costs to patients and the institution while providing personalized care is highly desirable.
