The Chronic Care Management Navigator (CCN) works in collaboration with the primary care provider and all members of the healthcare team. Responsible and accountable for direct and indirect patient care for designated patient populations. Assesses healthcare needs and identifies community resources. Serves as patient advocate and assists identification and improvement of service delivery. The CCN is responsible for facilitating care coordination services for patients in need of enhanced care coordination services. Eligible patients will be ensured that they will receive timely, high quality, and efficient health care and support services through the development of a multi-disciplinary care plan, self-management goals, and referrals to both internal and external resources. The CCN also serves as a clinical resource to other clinical staff.
REQUIREMENTS
1) Bachelor's degree in a health related field or RN with a minimum of 12 months experience in health care setting OR Associate's degree in health related field or LPN with a minimum of 36 months experience in health care.
2) Ability to obtain Chronic Care Professional (CCP) Certification within 24 months.
3) Ability to work independently and exercise clinical judgement in interactions with Physicians, payers, patients, and their families.
4) Strong organizational and time managements skills.
5) Strong analytical and data management skills.
6) Aware of scope for practice boundaries, comfortable seeking direction and assistance from appropriate resources.
7) Communication- Ability to communicate effectively verbally and in writing. Excellent negotiation skills.
8) Computer Skills - Proficient ability to use a computer and electronic medical record.
9) Confidentiality - Maintain patient, team member and employer confidentiality. Comply with all HIPAA
regulations.
10) Customer Service Oriented - Friendly, cheerful and helpful to patients and others. Ability to meet patients and
others needs while following office policies and procedures.
11) Delegating - Ability to allocate authority and/or task responsibility to appropriate team members, peers or
superiors.
12) Flexibility - Ability to adapt easily to changing conditions and work responsibilities.
13) Positivity - Display a positive attitude and is a positive agent for change.
