Scope of Position
Staff Pharmacy evaluates medication use, approval, production and preparation, dispensing and progress of therapy. Prepares and maintains all records on prescriptions and patient charges. Advises and communicates to other health care professionals about safe and effective medicine use.
Position Summary
Responsible for all aspects of staff pharmacist role with minimally required pharmacy personnel and adjusted workflows. Verifies new medication orders, checking prepared medications, overseeing dispensing from the pharmacy, and completing clinical patient review for safety and accuracy. Provides safe, efficient, high quality, patient-centered, and evidence-based medication services to all patients. In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family members, pharmacists, nurses, physicians, advanced practice providers, medical students, undergraduate and graduate pharmacy students, and other associated hospital personnel. Screens for drug-drug, drug-allergy, drug-disease state, and drug-laboratory interactions. Obtains and maintains clinical privileges, if applicable. Using clinical privileges, adjusts medications based on route of medication administration access, monitors and adjusts medications based on renal, hepatic, antithrombotic indications, and hematologic parameters. Participates and provides support in the Emergency Response effort, including Code Blues and Stroke Alerts.
Minimum Qualifications
Bachelor's Degree in Pharmacy. Registered Pharmacist. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
