Overview

Patient management – Uses EPIC for report writing to reach out to patients by making cold calls, My Chart messages, sending letters. Supports the team by entering referrals, orders and interacting with the care team to ensure health maintenance and preventive care is provided. Scrubs (reviews and manages) charts daily to understand what is and is not needed by each scheduled patient. Proactively schedules appointments.


Care coordination – Assumes a lead role in the work for health maintenance and panel coordination. Assists with questions regarding referrals and tests. Ensures patients and/or caregivers follow through with outside referrals to specialties, labs and diagnostic imaging. Coordinates hospital admissions and ancillary services needed for continuation of care and health maintenance needs. Works with care facilities, OHSU mission control and PMC to ensure that protocols are being followed for institutionalized and special needs patients.

Communication – Provides information and education to patients and gathers information for specific visits. Prioritizes and responds to patient questions via MyChart, phone, email and fax regarding scheduling and care issues within scope. Escalates all issues outside scope as appropriate.


Documentation/Data management – Documents legibly, concisely, correctly and completely to ensure team can accurately assess the status of care needs. Uses approved clinical resources and follows procedures appropriately. Participates as assigned with database input. Issues patient safety alerts to providers and helps manage the patients affected. Monitors department tracking tools and provides providers with real-time updates as appropriate. Initiates and implements processes to facilitate continual improvements related to data collection.



Responsibilities

  • High School diploma or equivalent, AND

  • Three years’ experience in a hospital or hospital-based clinic setting, AND

  • Current BLS certification at hire, AND

  • One of the following four:

    • 1. Completion of a nationally recognized accredited medical assistant training program, including a practicum (externship) of at least 160 hours, OR
    • 2. Successful completion of a formal medical services training program of the United States Armed Forces, OR
    • 3. Current Oregon Emergency Medical Technician (EMT) license (basic or advance) and national EMT registration with the National Registry of Emergency Medical Technicians (NREMT), OR
    • 4. Current Oregon Practical Nurse License
  • Ability to perform the job duties with or without accommodation.


Qualifications



  • Demonstrated proficiency working in an EHR, Microsoft Outlook, Word, and Excel.

  • Two years of work experience in a medical office setting, including high volume direct patient contact in both front and back office.

  • Philosophy and values consistent with a patent centered care model.
