PURPOSE:



The Jewish Board's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.



POSITION OVERVIEW:



The Grand Concourse Counseling Center is a CCBHC providing direct care to over 2200 clients annually, both on-site and in the community. The patient Navigator is a member of the Facilitated Intake Team and will act as a link between clients and the range of services available through The Grand Concourse Counseling Center. The Patient Navigator will engage individuals with complex behavioral health needs to develop person-centered care plans, provide care management services, track and arrange appointments, educate clients on other aspects of clients' health and community services.


The Navigator/Case Manager will act as a link between CCBHC clients and the range of services available at the CCBHC, agency and with community providers. S/he will engage clients and help link them to services and supports that address concrete needs (i.e., lack of medical care, food shortages); provide short-term case management services to address immediate barriers to care and coordinate services.



KEY ESSENTIAL FUNCTIONS:



  • Work collaboratively with clients and treatment providers to assist clients with rapid access to care and services.

  • Help clients navigate the intake process and, as appropriate, assist clients in understanding their diagnosis, treatment options and resources available.

  • Provide outreach to clients to facilitate engagement in services.

  • Help clients to address barriers to care

  • Prepare reports of community engagement contacts.

  • Function as an internal resource for clinical staff for the research and identification of client appropriate community based services.

  • Provide resources and referrals to clients as appropriate.

  • Complete documentation within required time frames and in required systems.

  • Attend scheduled team meetings and supervision.

  • Participate in staff development and training activities

  • Perform other responsibilities and duties as assigned.




CORE COMPETENCIES for the position include:


  • Good interpersonal, organizational, problem solving, and communication skills.

  • Effective advocacy skills including ability to work collaboratively with internal and external providers.

  • Basic computer skills




EDUCATIONAL/TRAINING REQUIRED:



  • Bachelor's degree in human service or public health field required.




EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:



  • Knowledgeable of mental health and social services and a commitment to providing person centered care




COMPUTER SKILLS REQUIRED:



  • Proficient with electronic health record; Microsoft Office (Word, Powerpoint, Excel, Outlook); comfortable and proficient using audio video platforms (i.e., Zoom).




VISUAL AND MANUAL DEXTERITY:



  • The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs.




WORK ENVIRONMENT / PHYSICAL EFFORT



  • Hybrid work setting. Position entails a combination of in person and remote work. Allocation of time and locations to be developed in collaboration with Executive Program Director and is based on the needs of the programs and clients served.

  • Travel required between program locations and other Jewish Board sites.
