PURPOSE:


Our client's Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally affirming, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities.

The ACT team provides an integrated set of evidence-based treatment, rehabilitation, case management, and support services delivered by a multi-disciplinary mental health treatment team. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. ACT services are developed through the person-centered service planning process and adjusted through the process of daily team meetings.

POSITION OVERVIEW:


ACT is a client centered/recovery focused treatment approach, where services are largely delivered outside of the office setting. Many ACT clients are at risk for psychiatric hospitalization, and require extensive supports in order to live independently in the community.

The position requires the application of specialized knowledge in the area of behavioral health supervision, diagnosis, documentation, substance use disorders, multidisciplinary team care, crisis services, complex care coordination, intensive treatment models, and fidelity programming.

This multi- faceted position works directly with adults living with severe and persistent mental illness (SPMI) and substance use disorders (SUD) living in NYC.

In addition to providing direct care, this position provides administrative oversight and support to the treatment team to ensure the delivery of services is in accordance with regulatory and payer requirements.

Clinical Coordinators use a recovery oriented approach to link recipients to the services they need to obtain housing and to achieve wellness self-management and increasing independence.

Working in a team setting and primarily in the community, Clinical Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate clients and coordinate other aspects of clients' health and community services.

ACT team members do a rotation of carrying an on-call phone on evenings and weekends, to be available to the crisis team for consultation regarding ACT clients.

KEY ESSENTIAL FUNCTIONS:

  • Clinical leadership and supervision.
  • Work closely with team to visit each client a minimum of 6 times monthly.
  • Provide all face to face, in person visits in the community, such as client's homes, shelters, residential programs, hospitals or other locations as client prefers.
  • Conduct comprehensive psychosocial assessments, assess for risk for substance use, suicidal ideations and aggression, and treatment plans, laying the groundwork for targeted interventions.
  • Use evidence-based treatments in the areas of mental health and substance abuse to address client needs-(e.g., cognitive-behavioral therapy, harm reduction, motivational interviewing, trauma-informed-care.
  • Assume administrative tasks such as chart auditing, collaborating with outside partners, facilitating team meetings and being the point person in crisis situations when Program and Team Director are not available.
  • Supervise students/staffAssist with on-boarding/orientation of new staff. When needed/requested, lead team in-service training on topics that will improve service to clients.
  • Integration of medical, specialized and behavioral health services in addition to social support and/or educational support services.
  • Periodic reassessment of a client's medical and behavioral health needs and update of, adherence to recommended treatments, and need for care coordination.
  • Attend daily team meetings, weekly supervisions, and mandatory trainings, actively contributing to collaborative efforts and staying at the forefront of best practices.
  • Attend and complete mandatory ACT trainings and Jewish Board MKSEI course training.
  • Demonstrate exceptional communication skills, facilitating effective collaboration within the multi-disciplinary ACT Team.
  • Exhibit strong engagement skills, ensuring meaningful connections with clients in diverse settings, including their homes and communities.
  • Adapt and respond effectively to the evolving needs of clients, promoting a holistic and dynamic approach to mental health care.
  • Collaborative development of an Individualized Care Plan (ICP) with the client, the client's family and/or caregivers in addition to other service providers, and review and revise ICP per regulatory standards or when there are significant changes in functioning or clinical status.
  • Provide services to assist clients with needs such as family support, housing, entitlements, employment, education, vocational interests, community integration, and connection to physical healthcare.
  • Escorting clients to various appointments such as medical appointments, HRA, court, job interviews, grocery stores and other as needed.
  • Following OMH regulations in regards to incident reporting, AOT monitoring and CAIRS
  • Provide after-hours emergency coverage via cell phone on a rotating basis, demonstrating a commitment to continuous support and crisis intervention
CORE COMPETENCIES:
  • Must be comfortable with the use of computers to maintain Electronic health record.
  • Must be a team player with excellent communication and time management.
EDUCATIONAL / TRAINING REQUIRED:

  • Master's Degree in human services with appropriate NYS licensure (e.g. MSW, LMSW, LMHC, LCSW, LMFT, Creative Arts Therapist LCAT, licensed Occupational therapist, Certified Psychiatric Rehabilitation Counselor, Rehabilitation Counselor, licensed psychologist)
OR
  • Hold a Master's Degree as a Therapeutic Recreation Specialist or Psychologist or Pastoral Counselor or Master's degree in a related field.
  • At least 1-year experience working on or with ACT team or similar type of community based/ mobile based program.
  • Multi Lingual preferred, ability to speak, write and understand the English language.
WORK ENVIRONMENT / PHYSICAL EFFORT:
  • Must be comfortable with public transportation, traveling in the community, as this position requires 80% of the time in the field and 20% in the office.
  • This individual must be comfortable walking and standing for long periods of time.

PAY RANGE AND BENEFITS:

Pay Range*: $75-79K
*Pay range offered to a successful candidate will be based on several factors, including the candidate's education, work experience, work location, specific job duties, certifications, etc.
