General Summary



Under general supervision, will work with the OPAT (outpatient parenteral antibiotic therapy) team, with some of the following responsibilities:



Principal Responsibilities and Tasks



The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.


Care coordination -


  • Screening new OPAT starts with checklist

  • Arranging for PA for medication for new outpatient starts

  • Communication of order changes to home infusion company (post-discharge; CM to do inpatient coordination still)

  • Extension or modification of antibiotic orders (end date, drug changes)

  • Triaging of THRIVE messages/patient calls

  • Weekly check-in call (scripted - any issues with line, issues with refilling medications triage to pharmacist/RN/MD)



Lab Safety --

  • Lab retrieval (LabCorp, fax, calling home infusion companies)

  • Lab entry/upload into Epic via Haiku

  • Triaging of abnormal lab values to MD

  • Call to home infusion pharmacy for addition of labs (CK, vanc trough, etc)


List Maintenance --

  • Daily review of active lists/update of specialty comments

  • Daily review of OPAT pre-list for new pts needing OPAT checklist review

  • Participation in once weekly OPAT rounds

Qualifications

  • High school diploma or equivalent is required.

  • Successful completion of a Pharmacy Technician Training Program accredited by the American Society of Hospital Health System Pharmacists and National certification required.

  • Maryland BOP registration required.

  • Successful completion of the departmental IV training and certification.

  • Three years applicable experience.

  • Ability to perform mathematical operations required in recording and counting of inventory, processing supply requests and for record keeping purposes, including addition, subtraction, multiplication and division. Ability to follow written and verbal instructions.

  • Ability to stand and/or walk for extended periods of time while performing pharmacy service support duties. Ability to lift supplies and equipment up to 40 lbs., up to approximately 40% of the time, while performing storage responsibilities, is required.

  • Highly effective verbal and written communication skills are necessary in order to work with medical, nursing and various allied health staff in order to successfully provide optimal patient care
