What You'll Do:

• Assess residents' physical conditions and develop individualized treatment plans


• Provide therapy sessions focused on improving mobility, strength, and function


• Collaborate with the interdisciplinary care team to ensure holistic resident care


• Educate residents and families on exercises and recovery plans


• Maintain accurate and timely documentation

What We're Looking For:

• Active Physical Therapist (PT) license


• Strong clinical skills and a passion for resident-centered care


• Excellent communication and teamwork abilities


• Experience in a post-acute or skilled nursing setting (preferred but not required)
