SUMMARY
The Data Entry Specialist will be responsible for accurately entering, updating, and managing healthcare data, including patient-related data, across multiple electronic medical record (EMR) systems. This role is critical in ensuring data integrity, supporting patient alignment initiatives, and assisting in patient-tracking processes. The ideal candidate will have excellent attention to detail, the ability to navigate complex data systems, and a basic understanding of healthcare data requirements. Additionally, responsibilities may include working across multiple EMRs, or other healthcare systems as needed to support Kidneylink.
Essential Duties and Responsibilities include the following. Other duties and tasks may be assigned.
Data Entry & Management:
Extract and enter patient data from EMR systems to support various clinical and administrative processes.
Track and document completed patient visits, including Chronic Disease Management Visits (CDMVs).
Update patient demographic information and maintain a record of upcoming appointments.
Patient Flagging & Alignment:
Identify and flag high-risk patients based on specified criteria to support targeted outreach and care management.
Implement new patient alignment flagging processes in EMR systems as directed by clinical or administrative guidelines.
Form Management & Uploading:
Prepare and send forms, such as PAM (Patient Activation Measure) and PHQ-9, for upcoming patient appointments.
Upload completed forms into designated systems to ensure they are available for clinical review and reporting.
Data Extraction & Reporting:
Assist with data extraction from multiple EMR systems to provide reports required by clinical and administrative teams.
Support ongoing data quality initiatives by identifying discrepancies and assisting with data cleansing as necessary.
Coordination with Clinical Teams:
Work closely with clinical teams to ensure accurate scheduling and documentation of patient visits.
Provide manual tracking support where automated solutions are not yet implemented, ensuring seamless data flow.
System Navigation:
Utilize multiple EMR systems to enter, verify, and extract data to meet the organization's reporting and operational requirements.
Update and manage patient identification numbers across systems to maintain data consistency.
Ad-Hoc Responsibilities:
Respond to ad-hoc requests, providing data entry, report generation, or administrative support to meet urgent needs.
Assist with special projects or new initiatives requiring data entry expertise, ensuring timely and accurate completion.
Collaborate with team members to troubleshoot issues or develop new processes to enhance data management practices.
Actively promotes GUEST customer service standards; develops effective relationships at all levels of the organization.
Participates in team concepts and promote a team effort; performs duties in accordance with company policies and procedures.
Regular and reliable attendance is required for the job.
