Job Title: Master Social Worker (MSW) Spanish/English Speaking
Position: Community Integration Counseling (CIC)
Location: New York
Hour: Per diem
Hourly Rate: $30-35 per hour

Position Overview:
We are seeking a dedicated MSW (Master of Social Work) to join our team and
provide Community Integration Counseling (CIC) Services. The MSW will work with
individuals directly to develop and implement person-centered plans that support
the individual's goals and preferences, and promote independence and community
integration.
Responsibilities:
? Conducting an initial assessment to understand the individual's strengths,
needs, and goals for community integration
? Developing a person-centered plan that outlines the support needed to help
the individual engage more fully in their community and improve their overall
quality of life.
? Providing counseling services to help individuals manage mental health
conditions, behavioral challenges, or other issues that may hinder community
involvement.
? Life skills training, such as social skills, communication skills, and
problem-solving techniques, to help individuals engage effectively in community
settings.
? Connecting individuals with local community resources, such as vocational
programs, recreational activities, educational opportunities, or social groups.
? Encouraging participation in community events and activities, fostering
relationships, and helping individuals navigate their environments in a way that
promotes independence.
? Facilitating access to vocational training or supported employment opportunities
that help individuals enter or re-enter the workforce.
? Advocating on behalf of individuals to ensure they have access to the services,
programs, and support they need in the community.
? Supporting family members or caregivers with information and resources to help
them provide effective care and encouragement.
? Developing and implementing crisis intervention strategies when needed,
ensuring that individuals have the support they need to stay safe and continue
progressing.
? Referring clients to other specialized services as needed, such as mental health
treatment, addiction services, or specialized rehabilitation.
? Maintaining thorough records of client progress, goals, and service plans to
ensure compliance with New York State regulations.
Qualifications:
? Bachelor's degree in social work, psychology, or a related field.
? Two (2) years of experience providing adjustment related to counseling to
seniors and/or individuals with physical and/or cognitive disabilities and
their families to be considered qualifying experience.
? Knowledge of the NHTD/TBI program and regulations.
? Strong communication and interpersonal skills.
? Ability to work independently and as part of a team.
? Proficiency in computer skills, including electronic health record
Systems. Valid driver's license and reliable transportation.
