The primary purpose of this job is to lead approval and auditing activities and assist with statistical analysis and compliance activities within Bridge Home Health and Hospice. This position also provides ongoing guidance and education to clinicians in accordance with all current applicable federal, state and local standards, guidelines and regulations and other requirements of all home care regulatory bodies.
Duty 1: Under the direction of Bridge leadership, oversees and participates in The Joint Commission accreditation preparation in utilizing audit outcomes, past survey experiences, and other identified areas for improvement.
Duty 2: Coordinates and collaborates with leadership and performance improvement team members in all aspects of Quality Assessment/Performance Improvement (QA/PI) Program and other review processes to ensure quality patient care, including ADR response and routine chart audits.
Duty 3: As directed, aggregates, submits, and tracks internal quality measures to outside sources for the purpose of benchmarking. Uses benchmarked data to collaborate with leadership and team members for education opportunities.
Duty 4: Utilizes software program to help interpret actual and projected publicly reported data and assists to abstract, track and trend data. Utilizes trended informationto identify areas for improvement.
Duty 5: Works collaboratively with the Episode Management Nurse, Performance Improvement Coordinator, Director of Quality, Intake Department, and interdisciplinary team (IDT) to review and ensure that necessary documentation for both Home Health and Hospice is in accordance with all currentapplicable federal, state, and local standards, guidelines, and regulations and is completed in accordance with expected timeframes.
Duty 8: Documentation is consistent with or exceeds Bridge Home Health and Hospice guidelines.
Duty 9: Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction.
Duty 10: Complies with organization policies, procedures, and practices to ensure quality job performance and Conditions of Participation from Medicare and Medicaid.
REQUIRED QUALIFICATIONS
Licensed by the Ohio Board of Nursing as a Registered Nurse
Minimum of 2-3 years of general nursing experience
Knowledge and commitment of the philosophy, mission, and goals of the agency and standards and regulations governing it
Excellent physical assessment, communication and documentation skills required
A valid driver's license is required (if you do not have a valid Ohio driver's license you must obtain one within 30 days of your residency in the state). You must also meet BVHS's company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.
Positive service-oriented interpersonal and communication skills required.
Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status. Must be able to interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the area's policies and procedures.
PREFERRED QUALIFICATIONS
1-2 years of skilled Home Health to including OASIS and Hospice including HIS
Previous experience abstracting and analyzing data
This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing. The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day. The individual must be able to lift twenty to fifty pounds and reach work above the shoulders. The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks. The individual must have excellent verbal communication skills to perform daily tasks. The associate must have corrected vision and hearing in the normal range. The individual must be able to operate a motor vehicle for business travel and community involvement.
