On-going training for program staff and fidelity of implemented therapeutic plans (OT, Dining. Supportive and protective devices, etc)
Monitor for and communicate changes in person's served skill performance
Liaison between programs and clinical staff around durable medical equipment including on-going inventory of durable medical equipment status across the division.
Be aware of, document, and communicate supply needs for therapeutic programs.
Use regular (weekly) supervision to determine upcoming schedules, review therapeutic programs and give input, etc.
Ensure that therapeutic programs are delivered in a culturally responsive manner that optimizes health, safety, independence and empowerment through the principles of positive behavioral supports and agency policies and regulatory standards
Other duties as assigned by supervisor and agency leadership
Requirements for the Position:
High school equivalency and at least 2 years of experience in the human service field.
Certifications: CPR/SFA, within 30 days of hire, crisis prevention & DPH MAP Certification within 90 days of hire, Driver's License upon hire
Proficient in clinical and professional verbal, writing, and computer skills.
Ability to go up and down stairs and the physical capacity to physically assist persons served as needed.
Use of personal cell phone and/or vehicle
