Responsibilities:
Evaluate patients within 24-hours or one business day of physician referral; document any delays in the medical record.
Develop and implement effective treatment plans, obtaining approval from the referring physician.
Administer treatments according to the physician's treatment plan.
Train and support the nursing department in the use of Restorative Aides.
Supervise Physical Therapy Assistants in patient care and related activities in compliance with state practice regulations.
Communicate patient progress, issues, and plans with supervisors and other healthcare team members.
Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences, as needed.
Conduct in-service training programs for facility staff.
Document evaluations, daily treatment notes, weekly progress notes, recertifications, and treatment changes as per state practice acts and payer requirements.
Educate patients' families and nursing staff on maintenance programs and caregiver training for discharge preparation.
Contribute to discharge planning and recommend necessary durable medical equipment to promote patient independence (e.g., braces, walkers, canes, grab bars).
Ensure department equipment is in proper working order by reporting any issues.
Establish positive, effective, and professional relationships with patients, families, staff, and colleagues.
Qualifications:
Licensed Physical Therapist in the applicable state; must comply with state practice acts.
Strong communication, interpersonal, and organizational skills.
Ability to work collaboratively as part of a healthcare team.
