Key Responsibilities:
Patient Care & Coordination:
- Provide direct patient care, including assessments and medical procedures, under RN supervision.
- Follow physician orders and the nursing process to manage patients' total needs.
- Coordinate patient care with the RN, therapists, and other team members, ensuring continuity of care.
- Report changes in patients' conditions and contribute to care plan adjustments.
- Meet established productivity standards and deliver patient-centered care.
Patient & Caregiver Education:
- Educate patients and caregivers on self-care, promoting independence and safety.
- Actively monitor and report any safety concerns to appropriate authorities or leadership.
Communication & Documentation:
- Complete accurate and timely documentation according to regulatory guidelines.
- Actively participate in care team meetings and contribute to collaborative problem-solving.
Staff Education & Professional Growth:
- Stay current with clinical knowledge by attending continuing education, in-services, and staff development activities.
Additional Responsibilities:
- Participate in special projects and assist with other duties as assigned by leadership.
- Comply with all Wellstar Health System policies, procedures, and standards of care.
Required Qualifications:
Education:

High School Diploma or equivalent required.
Licensure:
-
Valid LPN license in Georgia required.
Experience:
-
Minimum of 2 years of medical/surgical nursing experience in an acute care setting is preferred.
Certifications:
-
Basic Life Support (BLS) certification from AHA required.
Skills & Abilities:
- Strong clinical assessment skills and attention to detail.
- Excellent organizational, time-management, and problem-solving abilities.
- Exceptional written and verbal communication skills.
