Job Description
Scope of Position/Position Summary:
The Patient Care Resource Manager, PCRM, facilitates Medical Oncology patients through the health care system to include pre-admission to post discharge coordination of care. This may involve both the inpatient and outpatient settings. The PCRM monitors the patient care process and collects data to evaluate and improve timeliness and appropriateness of care. The PCRM collaborates with physicians, nurses and other health care workers to provide comprehensive health assessment, treatment, and follow-up evaluation for patients and to communicate the plan of care. The PCRM is the central information source for third party payers, communicating the plan of care, medical necessity, and gaining approval for post discharge care. The PCRM maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization. The PCRMs works closely with their PCRM colleagues and provide coverage for each other, as needed.
Minimum Education Required
Bachelor's Level Degree or equivalent combination of education and experience with a Major in nursing
Required Qualifications
Bachelors degree required, Masters preferred. RN with five years recent clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations preferred.
