The primary role of the Wound Nurse is to ensure effective and efficient care is delivered as prescribed by the physician and in compliance with facility policies and procedures. This position plays a crucial role in maintaining the highest standard of care for our residents, aligning with current federal, state, local, and corporate standards, regulations, and guidelines. The Wound Nurse reports directly to the Director of Nursing (DON).

Essential Duties and Responsibilities:


  • Provide excellent communication and strong customer service to residents, fostering a compassionate and supportive environment.

  • Maintain an acceptable standard of nursing practice and uphold professional decorum.

  • Accurately document nurses' notes, ensuring they reflect the care provided and the residents' response to that care.

  • Administer resident care, including executing physicians' orders, providing medications, and performing treatments in a manner that respects residents' dignity and self-respect.

  • Monitor, observe, and assess wounds and pressure sores, notifying physicians promptly of any signs of infection, deterioration, or lack of healing progress.

  • Ensure appropriate wound care management services are included in care plans, including special mattresses and nutritional supplements as needed.

  • Assist in preparing and updating care plans for treatment-related issues.

  • Support physicians and healthcare providers during visits and examinations.

  • Communicate with physicians and other health professionals regarding resident care, treatment, and condition.

  • Notify family members and attending physicians of significant treatment-related issues, such as:

  • Sudden or adverse changes in skin condition or wounds.

    • Untoward responses to medications or treatments, or refusal/unavailability of treatment.

    • Treatment errors that present significant risk.

    • Inability to obtain or administer prescribed treatment-related medication, equipment, or services in a timely manner.



  • Participate in the orientation and ongoing training of nursing staff.

  • Report significant findings, changes in residents' conditions, or potential concerns to the RN Supervisor and/or Director of Nursing.

  • Provide reassurance and compassionate care to residents who may be upset or in need of emotional support.

  • Participate in facility surveys and inspections conducted by authorized government agencies, as requested by the Administrator, Director of Nursing, or Assistant Director of Nursing (ADON).

Supervisory Requirements:

  • Assist with the overall supervision of the nursing staff to ensure quality care and compliance with facility standards.

Qualifications:


  • Education and/or Experience:

    • Possession of a Nursing Degree from an accredited college or university.

    • A minimum of 2 years of nursing experience, preferably in a long-term care facility.



  • Licensure/Certification: Active nursing license in the state ((Specify LPN or RN as required)).

  • Strong communication and customer service skills.

  • Proficiency in observing, assessing, and documenting resident care and wound conditions.

  • Ability to work collaboratively with healthcare professionals, residents, and families.

  • Patience, compassion, and dedication to delivering high-quality care.

Work Environment:


  • Must comply with safety protocols, infection control standards, and confidentiality guidelines.

  • Position may involve physical demands such as prolonged standing, walking, and lifting to 25 pounds.
