A successful Staff Pharmacist must be passionate about healthcare and want to make a difference in the lives of others while acting as a mission-driven catalyst to help Livingston Community Health deliver the best quality of care and excellent service to our patients and their families.
The Staff Pharmacist is responsible for ensuring the safe, effective, and efficient dispensing of medications and providing high-quality pharmaceutical care to patients. This position involves collaboration with healthcare providers and staff to achieve optimal patient outcomes through medication management and education. The Staff Pharmacist will adhere to all applicable regulations, standards, and organizational policies to maintain compliance and promote patient safety.
Essential Functions, Duties, and Responsibilities
Pharmaceutical Care and Dispensing:
Verify and accurately dispense medications prescribed by authorized healthcare providers.Provide medication counseling and education to patients, ensuring proper use and understanding of prescriptions.Conduct medication therapy reviews to optimize patient outcomes and minimize risks.Maintain accurate and comprehensive records of all medication transactions and clinical interventions.
Collaboration and Communication:
Collaborate with healthcare providers to develop and optimize medication treatment plans.Act as a resource to the healthcare team, providing guidance on drug selection, dosage, and potential interactions.Attends and actively participates in all meetings (e.g., department meetings, program meetings, staff meetings) and other activities as required or assigned.
Regulatory Compliance and Safety:
Ensure compliance with all federal, state, and local regulations governing pharmacy operations.Monitor and report adverse drug reactions, medication errors, and other quality-related issues.Maintain accurate inventory records and oversee proper storage of medications.
Education and Training:
Educate patients and caregivers on medication adherence, potential side effects, and proper storage.Mentor and train pharmacy technicians and students as needed.Stay current with advancements in pharmacy practice, medications, and regulations through attending workshops/seminars as necessary for continuing education.
Operational Support:
Assist in maintaining pharmacy inventory, including ordering, stocking, and monitoring for expired medications.Perform quality control checks on prescriptions and ensure pharmacy equipment is maintained.Support pharmacy initiatives, such as immunization programs and medication therapy management (MTM) services.Develop and implement innovative programs to improve medication adherence and chronic disease management.Stay informed on advancements in pharmacy practices and technologies to enhance service delivery.
Supports the overall needs of the health clinics by working flexible or extended hours when necessary. Demonstrates competence with the mission, vision, and values of the organization to provide quality healthcare to those served in the community. Other work-related duties as assigned. Education, Knowledge Skills, and Abilities
Education and Experience:
Graduate of an accredited School of Pharmacy with a B.S.Ph. (Bachelors Degree of Science in Pharmacy). A Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy is preferred.Experience working in an FQHC or similar healthcare setting is preferred.Experience in a retail, hospital, or community pharmacy setting is a plus.
License/Certification:
Must possess a current California Board of Pharmacy license in good standing Certification or expertise in 340B program management is preferred. Basic Life Support (BLS) - American Heart Association certification required Current Vaccinations
Knowledge, Skills, and Abilities:
Strong knowledge of pharmaceuticals, medication therapy, and clinical pharmacy practices. Excellent communication and interpersonal skills to effectively interact with patients and healthcare professionals. Detail-oriented with the ability to manage multiple tasks and maintain accuracy under pressure.Proficiency in pharmacy management systems and electronic health records (EHR).Commitment to serving underserved populations and addressing health disparities.Ability to provide effective guidance and training to subordinates supporting their success.
