BCForward is looking for Care Manager (RN)- Remote- IN

Position Title: Care Manager (RN)





Location: Indiana remote worker, with occasional in-office attendance for staff meetings (quarterly) at 429 N. Pennsylvania, Suite 109, Indianapolis, IN, 46204.




Anticipated Start Date: 1/27/2025




Expected Duration: 01 Year Contract




Job Type: [FULL TIME (>=40 HRS WEEKLY)/ [TEMP], [Remote]




Shift: 8:00 AM - 5:00 PM, with potential for alternate shifts after 120 days based on performance.




Pay Range: $45.00 - $50.00/hr on W2




Position Purpose:

The Care Manager (RN) is responsible for developing, assessing, and facilitating complex care management activities for members, primarily focusing on physical health needs. The role aims to ensure high-quality, cost-effective healthcare outcomes by providing personalized care plans, education, and support to members and their families. The Care Manager evaluates the needs of each member, addresses barriers to care, and helps facilitate the best care plans in collaboration with providers, specialists, and community resources.



Key Responsibilities:




  • Care Management and Coordination:


    • Assess the physical, social, and healthcare needs of members.

    • Identify and address barriers to accessing appropriate care, including social determinants of health.

    • Develop and update personalized care plans, collaborating with providers, specialists, and community resources.

    • Coordinate care between members, caregivers, and the healthcare team to ensure high-quality, person-centered care.

    • Monitor and evaluate member progress toward care plan goals and update care plans as necessary to meet evolving needs.



  • Care Plan Development:


    • Create ongoing, personalized care or service plans tailored to member needs and priorities.

    • Collaborate with healthcare providers to ensure timely and appropriate care delivery.

    • Offer educational support to members and caregivers on disease management, healthcare options, and available community resources.



  • Resource Support:


    • Identify and provide referrals to resources that address needs such as housing, employment, or social support services.

    • Facilitate connections between members and community resources to ensure comprehensive care.



  • Communication and Outreach:


    • Perform outreach via telephonic, digital, or in-person assessments to evaluate member needs.

    • Provide telephonic or digital engagement through cold calls to enroll members in care management programs and develop care plans.



  • Documentation and Compliance:


    • Maintain comprehensive, accurate, and timely documentation of all care management activities.

    • Ensure compliance with state, federal, and third-party payer regulations related to care management and healthcare delivery.



  • Education and Training:


    • Provide education to members and caregivers about healthcare benefits, disease management, and care options.

    • Continuously provide feedback to leadership on opportunities for improving care and quality delivery.





Skills/Experience:




  • Required:


    • Strong clinical knowledge and experience in nursing, particularly in adult care and medical/hospital settings.

    • Proficient in using multiple computer programs and electronic health record (EHR) systems.

    • Excellent verbal and written communication skills.

    • Ability to engage members in a positive and empathetic manner, especially in telephonic or cold-call settings.

    • Registered Nurse (RN) licensure in the state of practice (or Compact State Licensure).



  • Preferred:


    • Experience with Medicaid insurance and Managed Care.

    • Care management or case management certification (CCM) preferred.

    • Experience with telephonic or digital outreach and cold calling for member engagement.





Education/Experience:




  • Required:


    • Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.

    • 2-4 years of related experience in nursing, care management, or a clinical setting.



  • Preferred:


    • Experience in care management or managed care settings.

    • Knowledge of Medicaid and insurance processes is preferred.





Performance Metrics and Expectations:



  • Maintain a caseload of approximately 50 members.

  • Complete 20-40-member outreach calls daily.

  • Achieve a 90% monthly Quality Audit score.
