Major/Essential Functions
Employee will be required to provide mental health counseling services to patients of the TTUHSC SON Combest Central Community Health Center (CCCHC). Work involves being a member of a healthcare team that provides healthcare to patients which include the following duties and responsibilities:
Provide mental health assessment and evaluation, as well as formation of individual treatment plans for patients
Provide appropriate interventions to patients referred by primary care providers
Conduct interviews to gain information about the individual's life to determine current mental functioning, stability for procedures and develops formal assessments for use of treatment plan formulation
Provide individual and group educational activities to patients and deliver mental health services
Collaborates with patients to develop individualized program of health promotion and self-management.
Educate patient about health care and treatment options, programs, and resources
Initiate the coordination, scheduling and delivery of mental health services
Participate as an active member in the inter-professional collaborative practice team (IPCP)
Ensure compliance with specialty area licensing Board, TTUHSC policies, and departmental policies and procedures
Ability to communicate effectively with a diverse patient population
Meet patient data entry timelines in the EHR system prescribed by the institution and department
Meet departmental productivity standards

Preferred Qualifications
Demonstrate working knowledge of confidentiality and understand federal laws related to HIPPA
Experience documenting and retrieving information in electronic health records (EHR)
Experience managing patient caseloads and submitting patient charting by deadline identified by the department
Experience with software EHR, Word, Excel and Outlook applications
Experience generating patient data for grant reporting requirements, PI, QI, and assessment
Communicate effectively in English and Spanish to meet patient needs of the clinic
Experience making patient referrals
Experience organizing data concerning patient's family and medical history, and onset of symptoms obtained from patient, relative, and other sources such as other medical health related professionals

Required Qualifications
Education: Master's degree in psychology, counseling, social work, or related field.
Experience: Three (3) years of related experience required
Licenses/Certificates (one of the following):
* Licensed Professional Counselor (LPC) * Licensed Professional Counselor - Associate (LPC - A) * Licensed Clinical Social Worker (LCSW) * Licensed Master Social Worker (LMSW) must be working towards LCSW * Licensed Marriage and Family Therapist (LMFT) * Licensed Marriage and Family Therapist - Associate (LMFT-A) * Licensed Psychological Associate (LPA)
