Job Title: Admissions Nurse

Job Summary
We are seeking a compassionate, organized, and experienced Admissions Nurse to join our team at a Skilled Nursing Facility (SNF). The Admissions Nurse will play a pivotal role in ensuring a seamless and efficient admission process for new residents, collaborating with physicians, interdisciplinary teams, and families to assess patient needs and ensure the highest standard of care. This role requires a combination of clinical expertise, excellent communication skills, and a deep understanding of regulatory guidelines.

Key Responsibilities



  • Resident Admissions:

    • Manage the admission process for new residents, including reviewing medical histories, conducting initial assessments, and completing required documentation.

    • Collaborate with physicians, families, and interdisciplinary teams to ensure that all medical, personal, and social information is accurately gathered and understood prior to admission.

    • Ensure that the necessary medical records and insurance documentation are completed and processed in a timely manner.




  • Clinical Assessment & Care Planning:

    • Conduct thorough clinical assessments for incoming residents to identify medical needs, care requirements, and any specialized treatments.

    • Develop and implement individualized care plans in collaboration with the interdisciplinary team, ensuring that all aspects of care (medical, physical, emotional, and social) are addressed.

    • Monitor the admission process to ensure that each resident's transition into the facility is smooth and efficient.




  • Coordination & Communication:

    • Serve as the main point of contact between families, physicians, and the facility regarding the admission process.

    • Ensure effective communication between departments, including nursing, social services, dietary, and therapy, to ensure continuity of care.

    • Educate families and new residents on facility policies, care expectations, and available services.




  • Regulatory Compliance & Documentation:

    • Ensure all admissions meet the regulatory requirements set by the state and federal government, maintaining compliance with all healthcare regulations.

    • Complete all necessary paperwork, including but not limited to medical records, consents, and insurance information, ensuring accuracy and confidentiality.

    • Ensure compliance with facility policies regarding the resident admission process, including the verification of insurance coverage and obtaining necessary authorizations.




  • Patient and Family Support:

    • Provide emotional support to new residents and their families, ensuring they understand the care process and feel confident in their decision to choose the facility.

    • Address any concerns or questions that arise during the admission process and throughout the transition to the facility.




  • Collaboration & Teamwork:

    • Work closely with the facility's medical staff, including physicians, nursing staff, and therapy teams, to ensure the comprehensive care of each resident.

    • Participate in interdisciplinary team meetings, contributing insights into residents' medical conditions and care needs.




Qualifications


  • Education & Experience:

    • Registered Nurse (RN) or Licensed Vocational Nurse (LVN) with current licensure in good standing.

    • Minimum of 2 years of nursing experience in a healthcare setting, preferably in long-term care or skilled nursing.

    • Experience with resident admissions or healthcare intake processes is a plus.




  • Skills & Abilities:

    • Excellent clinical assessment skills and a thorough understanding of the care needs of elderly or medically complex residents.

    • Strong organizational and time-management skills, with the ability to manage multiple tasks and prioritize effectively.

    • Exceptional communication and interpersonal skills, with the ability to work collaboratively with residents, families, and multidisciplinary teams.

    • Proficiency in electronic health records (EHR) and other clinical documentation systems.

    • Knowledge of state and federal regulations governing long-term care and admissions procedures.




  • Certifications:

    • Current CPR/BLS certification required.

    • Additional certifications in geriatric nursing or case management are a plus.




Physical Demands

  • Ability to lift and/or move up to 25 pounds occasionally.

  • Prolonged periods of sitting and use of a computer.

  • While performing duties, the employee is regularly required to sit, stand, walk, talk, and hear.

  • Occasional travel may be required for home visits or community outreach.


Work Environment

  • The work environment is primarily clinical, with potential exposure to various infectious diseases, bodily fluids, and hazardous materials.
