Job Summary
Under the direction of the Ambulatory Safety Net nurse program manager, the clinical navigator, RN will support operations of a system-wide ambulatory safety net program including daily oversight of a team of patient navigators. This position requires leadership, sound clinical judgment, effective problem-solving skills, critical thinking, excellent organizational and interpersonal skills, some management experience, and the ability to multitask. The position requires collaboration with other teams within the Office of the Chief Medical Officer (OCMO) and with clinical departments across the Mass General Brigham Enterprise.



Qualifications




  • Contribute to the design and development of current and future system-wide ambulatory safety nets including


    • Colorectal cancer

    • Clinically significant radiology results (non-imaging follow up)

    • Prostate cancer

    • Breast cancer

    • Other ASNs as needs are identified



  • Support clinical consensus process for maintaining and initiating safety nets by closely partnering with clinical subject matter experts to implement protocols for prioritized conditions

  • Organize, lead, and facilitate internal team meetings and MGB site-level management and staff (e.g. Endoscopy and Urology) to ensure effective communication and collaboration. Prepare meeting agendas, provide updates on relevant policies and procedures, and address concerns and challenges.

  • Ensure the delivery of high-quality patient care by providing clinical oversight to patient navigators, offering guidance and support in complex cases through regular meetings and communications, and escalating issues to the nurse manager and/or medical director as appropriate.

  • Implement and manage quality and safety check process to regularly review patient cases and staff workflows.

  • Participate in the recruitment, hiring, and onboarding process of patient navigators.

  • Oversee radiology quality and safety initiatives by ensuring follow up recommendations are completed through a closed-loop system, addressing any issues identified, and collaborating with relevant teams.

  • Perform chart reviews in the electronic medical record (Epic) to determine whether important follow up care is needed and follows standard operating procedures to ensure necessary care is provided.

  • Consult with the patient's PCP, specialist physicians as well as other members of the care team as needed, regarding their patients.

  • Monitor and track relevant metrics, including pathology results, to ensure appropriate patient management and identify areas for process improvement.

  • Understand project scope and recognize when direction from nurse manager or medical director is needed.

  • Perform other duties as assigned.






This role is grant-funded through December 31, 2025, with the possibility of an extension.




Qualifications:



  • Graduate of an accredited school of nursing.

  • A registered nurse licensed to practice in Massachusetts.

  • BSN required.

  • Previous experience working in an ambulatory setting is preferred.

  • At least 3 years of nursing experience required.

  • Fluency in PC /Microsoft applications including Word, Excel, PowerPoint, Access, Adobe Acrobat

  • Experience with Epic EHR
