We are seeking a Service Coordination Staff member for our Adult Day Health Program. This role involves working directly with participants to ensure their physical, mental, and social needs are met through comprehensive service coordination and support.
Key Responsibilities: As part of the Service Coordination team, your duties will include:
Community Resource Referrals: Inform and refer participants to appropriate community services to enhance their well-being.
Comprehensive Assessments & Care Planning: Participate in developing individualized care plans, working closely with a multidisciplinary team to meet participants' needs.
Service Coordination Documentation: Maintain service coordination notes in participants' health records at the start of service, when significant changes occur, and at least once every 90 days or as needed.
Medically-Related Services Coordination: Facilitate coordination of physical, mental, and behavioral health services at enrollment, during care, and at discharge.
Support & Counseling: Assist participants with personal, social, family, or adjustment issues encountered during the program.
Specialized Counseling Referrals: Refer participants or families to community resources if specialized counseling services are required.
Inter-Agency Coordination: Ensure seamless service delivery by coordinating with community agencies when no other agency is designated as the service coordinator.
Holistic Care Management: Assess, plan, implement, coordinate, monitor, and evaluate services needed to support participants' overall care.
Qualifications:
Education: Minimum of a Bachelor's degree in human services or a related field from an accredited college or university.
Experience: At least one year of recent and relevant experience working with adults in a professional capacity.
Licensure/Supervision: Must meet the requirements under 258 CMR 12.00 (Scope of Practice) for licensure or appropriate supervision.
Skills: Strong organizational, communication, and problem-solving skills, with the ability to work collaboratively in a multidisciplinary team environment.
Work Authorization:
Must be authorized to work in the United States.
