Job Description:

The Food for All San Gabriel Valley (Food4All SGV)/Social Determinants of Health (SDOH) Initiative will be implemented under the leadership and guidance of the Health Consortium of Greater SGV/Heluna Health and the Greater SGV Hospital Collaborative. This initiative aims to connect vulnerable patients identified as food insecure or experiencing other SDOH challenges at hospitals and Federally Qualified Health Centers in the Greater SGV area with food resources and other SDOH services provided by local Community-Based Organizations (CBOs).

Reporting to the Health Consortium Director, the Program Coordinator will collaborate with program partners to support the development and expansion of program infrastructure and ensure effective implementation of project objectives. Their efforts will focus on sustaining the existing Food4All SGV program with hospitals while extending its reach to additional healthcare providers (hospitals and health centers) and addressing broader SDOH needs.

 

This is a part time, non-benefitted position at 24 hours per week.  This position is grant-funded through February 2026 with possible extension pending funding availability.

This position will require occasional local travel in the San Gabriel Valley area of Los Angeles County.

Employment is provided by Heluna Health.

 

The pay rate for this position is $33.00 per hour. 

 

KEY RESPONSIBILITIES 

Program Expansion and Outreach


  • Support the planning and execution of efforts to expand the Food for All SGV/SDOH program, including outreach to new healthcare and SDOH partners.

  • Coordinate the implementation of the expanded program initiatives.

Partnership Development and Relationship Management


  • Build and maintain relationships with staff from existing and new healthcare partners, fostering a shared vision for the program.

  • Engage with current food partners and establish connections with additional food partners and other SDOH service providers.

  • Serve as the primary point of contact for all program partners, promoting positive and collaborative relationships.

Stakeholder Engagement and Meetings


  • Assess the interest of healthcare providers in establishing connections with specific service types.

  • Schedule and organize presentations and develop agendas for regular coordination meetings with new and existing program partners (monthly or as needed).

  • Facilitate coordination meetings, to include information sharing, exchange of lessons learned, and partner check-ins to ensure effective linkages between healthcare organizations and SDOH providers.

  • Support addressing and resolving challenges identified during meetings.

System and Workflow Development


  • Work with healthcare staff and consultants to assess and enhance systems and workflows across hospitals and partner organizations, including patient screening, documentation of results, and referral processes.

  • Provide support and technical assistance to healthcare organizations and SDOH CBOs to establish referral protocols, expectations, and Memoranda of Understanding (MOUs).

Program Implementation and Evaluation


  • Review outcomes of program pilot tests and facilitate discussions to refine processes as needed.

  • Serve as the program liaison with findhelp to implement the platform at three new hospitals and monitor its usage for referrals to food and SDOH CBOs.

  • Collect and analyze data to support project evaluation and improvements.

Reporting and Documentation


  • Maintain accurate records and compile data to generate required reports.

  • Support preparation of progress reports to funders.

JOB QUALIFICATIONS

Education/Experience 


  • Bachelor’s degree in Healthcare Management, Health Administration, Health Systems Management, Public Health, or related field preferred.  Master’s degree preferred.

  • Background in community health and/or hospital systems.

  • Previous experience as a project or program coordinator working with multiple organizations.  

  • Experience with data management systems preferred.

Certificates/Licenses/Clearances


  • Background check required

  • A valid California driver’s license, proof of vehicle insurance, and reliable transportation or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions

WORK ENVIRONMENT


  • General Office Setting, Indoors Temperature Controlled. Local travel (by car) will be required once the program returns to the office.
