Scope of Position
In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel, provides direct patient care for patients of the Comprehensive Transplant Center which includes the acute care and ambulatory care settings as an Transplant Specialty Practice Pharmacist. Approves new drug orders and screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in the pre-, peri-, and post-transplant management of patients in the hospital and in the transplant clinic. Participates in appropriate hospital and/or departmental committees relevant to transplant.
Position Summary
This position functions to coordinate all aspects of patients' pharmaceutical needs in abdominal transplantation. Duties include but are not limited to: participation in the transplant clinic; medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; medication reconciliation; coordination of patients' pharmaceutical needs with physicians, mid-level providers, and nurses. The patient population includes adult kidney, liver, and pancreas pre- and post- transplant recipients. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for patients and hospital staff (physicians, nurses and pharmacy staff).
Minimum Qualifications
Bachelor's Degree in Pharmacy. Registered Pharmacist. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
