Redwood Cove Healthcare Center is Hiring an LVN MDS Coordinator

Redwood Cove Healthcare Center in Ukiah is looking for a detail-oriented and organized LVN MDS Coordinator to join our team. This is a full-time position with a competitive starting salary of $85,000 per year.

• Oversee the MDS (Minimum Data Set) assessment process to ensure accuracy and compliance with federal and state regulations


• Coordinate with interdisciplinary teams to develop and update resident care plans


• Conduct assessments and ensure proper documentation to support reimbursement and quality outcomes


• Monitor and manage resident assessment protocols (RAPs) and quality measures


• Collaborate with nursing staff and other departments to ensure residents receive appropriate care


• Stay up to date with changes in MDS regulations and provide staff education as needed

• Licensed Vocational Nurse (LVN) in the state of California


• Previous MDS experience preferred but willing to train the right candidate


• Strong organizational and analytical skills


• Ability to work collaboratively with interdisciplinary teams
