Education: Graduate of an accredited Licensed Vocational Nursing Program.
Experience: 1 year experience required. Preferred (3) years nursing experience.
Summary: The Licensed Vocational Nurse (LVN) uses sound medical and professional judgment and best practices to support the medical, safety and well-being of every resident while maintaining compliance with established state nursing and licensing regulatory agencies, and federal standards requirements. The LVN, working closely with their assigned QIDP, will be responsible for managing medical care of their designated residents to include, but not limited to well visits, immunizations, sick visits, documentation, medication audits, supply inventory, ordering and communication with parents/guardians.
The LVN PRN staff will perform all job duties as described during the assigned work hours, as needed.

Essential Functions:
1. Perform nursing duties in accordance with nursing guidelines and best practices established by the Texas Board of Nursing for Licensed Vocational Nurses
2. Perform the following duties, while working in conjunction with a QIDP, to ensure the highest level of medical care for each resident assigned to you:
a. Schedule and attend all medical appointments
b. Review PCC documentation daily
c. Respond to injuries and illness during normal business hours
3. Regarding medication for the residents assigned to you:
a. Manage medication ordering and delivery
b. Ensure accurate distribution of medications
c. Maintain adequate supplies of medication
d. Ensure proper storage of medications
e. Re-label any medication that is changed with updated label
4. Maintain medical documentation for each resident assigned to you
5. Appointment summaries are to be entered into PCC within 24 hours of the appointment, including any written material or oral instruction provided at appointment.
6. Update medication administration records (MAR) in real time when medications are changed, added or discontinued.
7. Create nurse progress note, in real time, in PCC when any communication regarding medical care is received from or provided to a resident, guardian, or Breckenridge Village staff member
8. Communicate in a clear, transparent, and timely manner all information regarding each resident assigned to you
9. Meet with assigned QIDP no less than weekly to discuss relevant medical information including but not limited to: medical appointments, illness, injury, non-emergent changes in medical conditions.
10. Contact the QIPD immediately when any of the following occur:
a. Illness or injury requiring transport to Urgent Care or Emergency Department
b. Illness that requires a medical appointment or isolation/quarantine
c. Injury that requires medical attention or
d. A medication change is ordered for resident.
11. Contact the parent/guardian within the same business day to discuss
a. Medical appointments, review documentation and instructions
b. Medication changes
c. Illnesses or injuries requiring transport to Urgent Care or Emergency Department
d. Illness that requires a medical appointment or isolation/quarantine or
e. Injury that requires medical attention
12. Participate in on-call rotation with LVNs, Nurse Manager and Chief Medical Officer to provide 24/7 Nurse on Duty (NOD) coverage to Breckenridge Village
13. NOD rotations begins at 4:00 p.m. on Friday and ends at 4:00 p.m. the next Friday.
14. Respond to all calls immediately, but no later than 10 minutes after call is received
15. Be available to respond to the medical needs of residents on campus as soon as possible
16. Have contact information readily available in the event notification is required.
17. Prepare and distribute written summary of all weekend communications no later than noon on Monday
18. Attend in-person or video conferencing meetings that involve any resident assigned to you, as needed.
19. Weekly consultation with Director of Nursing
20. Must be able to perform physical activity such as extensive walking, lifting of patients, stretching and bending
21. Report signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures
22. Adhere to professional and ethical standards of conduct outlined in Breckenridge Village personnel polity manual including demonstrating respect for agency staff, residents, and community members and complying with required dress code at all times
Measurable Deliverables:
1. Meet all state regulatory guidelines and standards that are applicable to this position, with no deficiencies or concerns regarding medical care during annual fundamental surveys.
2. Maintain resident physical and mental safety by making immediate report of any changes in resident health of behavior to the DON and the QIDP with whom you collaborate
3. Provide the highest level of resident care
4. Daily review of PCC and act as required
5. Daily review of communication from residents, parents/guardians, QIDP or any other team member and act as required
6. Address each medical concern with urgency
7. Treat resident medical needs with the utmost care and attention
8. In emergency situations, call 911 first
9. Proper time management of all daily, weekly, monthly, quarterly, and annual tasks
10. Document all resident health information as close to real time as possible but not later than 24 hours
11. Communication timeframes (during normal business hours): Answer all phone calls, texts, or emails regarding urgent residential medical needs or respond in a timely manner

Credentials, Skills, and Abilities:
1. Maintain current licenses and certificates required for the position including, but not limited to:
2. Good standing as a Licensed Vocational Nurse or Registered Nurse with the state of Texas Board of Nursing
3. Current BLS certifications and First Aid Certification
4. Pass a pre-employment drug screen and random drug screens throughout employment.
5. Posses a valid Texas Driver's License.
6. Pass a pre-employment and biennial criminal background checks.
7. Must be able to maintain professional people skills, oral, written, listening, and non-verbal communication skills
8. Demonstrate the ability to:
9. Respond sensitively and competently to the service population's cultural and socio-economic characteristics
10. Work collaboratively with other staff members, service providers, and professionals
11. Communicate effectively in writing and verbally in English.

13. Maintain computer literacy required to meet the responsibilities of the position
14. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team
