Position Summary: We are looking for a compassionate and skilled Certified Nurse Practitioner (NP) to join our provider network. In this role, you will perform in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary goal will be to evaluate overall health and well-being to ensure accurate and comprehensive risk adjustment coding, contributing to improved value-based care.



Key Responsibilities:

• Conduct annual in-home wellness visits and health assessments for members with chronic conditions.
  
  


• Perform comprehensive evaluations, including physical exams, body system reviews, vital sign assessments, and medication history reviews.
  
  


• Assess members' functional status, cognitive abilities, home environment, fall risks, mental health, and social factors.
  
  


• Provide patient health education on topics such as pain management and medication.
  
  


• Assist in closing care gaps by facilitating screenings and labs.
  
  


• Build trust with members through exceptional communication and empathy.
  
  


• Collaborate enthusiastically to enhance care delivery.
  
  

Qualifications:

• Valid, unencumbered NP license in the state of practice.
  
  


• Access to reliable transportation to travel within a 55-mile radius in the job posting county.

  • Previous experience in in-home risk assessments.
    
    
  
  
  • 3+ years of patient care experience (primary care/adult/geriatric, EMR).
    
    
  
  
  • Prior 1099 (PRN) experience.
    
    
  
  
  • Bilingual proficiency.
    
    
  
  
  
  


• Ability to work independently and effectively use EMR systems.
  
  


• May be requested to obtain additional NP licensure with support provided.
