Job Summary Under the direction of the Clinical Director of Care Management and within established organizational and departmental policies and procedures the Case manager coordinates with physicians, nurses, social workers, and other health team members to expedite medically appropriate, cost-effective Case.
The Case manager applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. Responsible for covering the Emergency Department, Surgical Day Care, Medical Day Stay, and Admitting.
This position is part of the Massachusetts Nurses Association Bargaining Unit at Cooley Dickinson Hospital.

Education
Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred

Licenses and Credentials
Registered Nurse [RN - Massachusetts]
Experience
One (1) to three (3) years' experience in discharge planning, utilization management and/or reimbursement 2-3 years required
Experience or training in critical pathways, clinical guidelines, outcomes management and demonstrated clinical expertise and experience with complex discharge planning required
Knowledge, Skills and Abilities
BSN required or must be enrolled in BSN program or actively in pursuit of BSN
Case Management certification desirable
Strong demonstrated knowledge of community health services required
Demonstrated knowledge of InterQual criteria preferred
Demonstrated proficiency with personal computers, with word processing as well as ability to learn MIDAS required
Demonstrated excellent interpersonal, organizational and communication skills and ability to collaborate effectively with physicians and other members of the health care team required
Demonstrated ability to orchestrate many activities and manage multiple priorities required.
