Job Summary: The Social Services Director is responsible for providing medically related social services to support residents in achieving the highest practicable level of self-care, independence, and well-being. This role involves coordinating social services, evaluating resident needs, and assisting with healthcare decisions, emotional support, and community resource coordination.
Key Responsibilities:
Assess residents' social service needs by evaluating social, family, psychological, and emotional factors.
Establish care plans by setting goals with residents and their families.
Provide emotional support and counseling to residents and families, addressing concerns such as coping with physical disabilities, health-related fears, and grief.
Assist residents with healthcare decisions and securing appropriate prosthetics or assistive devices.
Coordinate with interdisciplinary teams to ensure residents receive necessary services.
Assist in discharge planning, securing necessary services, equipment, and referrals.
Organize and track personal belongings, respond to reports of missing items, and assist with personal shopping.
Arrange outpatient appointments and on-site ancillary patient services (e.g., optometry, podiatry, dentistry, and psychiatric services).
Develop and maintain strong relationships with facility departments and external agencies to enhance social services.
Document resident social service status and maintain accurate records.
Provide in-service training to staff as needed.
Qualifications:
Bachelor's Degree in Social Work or Human Services required; Master of Social Work (MSW) preferred.
Licensed Social Worker
Minimum of 2 years of supervised social work experience in a healthcare setting.
Experience in a Skilled Nursing Facility is REQUIRED.
Knowledge and experience with PCC preferred.
Ability to read and comprehend technical procedures, policy manuals, and present information effectively.
Strong problem-solving skills with the ability to interpret various forms of instructions.
