Job Summary:




As a RN Coordinator at our pulmonary clinic, you will work closely with our providers and interdisciplinary teams to manage our pulmonary hypertension patient populations in addition to assisting with our interventional pulmonology program. This role is part of a collaborative team of nurse coordinators, where you'll gain invaluable experience in pulmonary medicine while making a difference in the lives of patients. Provides clinical management and coordination of care for program patients in collaboration with the multidisciplinary team. Provides feedback to department leadership on management of program resources, identifies and/or supports identified quality improvement initiatives, and provides continuum of care to the assigned patient population. Works with patients and families directly, along with a team of providers, inpatient colleagues and support staff, prior authorization team, payors and community resources. Care and services provided according to the Nursing Scope of Practice.



EXPERIENCE DESCRIPTION:



A minimum of three to five years of experience in nursing field is required. Patient care experience in related program setting is required. Experience in procedural care preferred.



EDUCATION DESCRIPTION:



Bachelor's degree in Nursing is required.



SPECIAL SKILLS DESCRIPTION:



High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative, problem solve and be proactive in assisting the care team in managing this patient population.



LICENSURE DESCRIPTION:



Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days of hire.
