Job Overview

Reporting to the Youth Services Grants Supervisor, the Youth Services Linkage to Care Coordinator (LCC) is responsible for referring Youth Services clients (Cal Learn, AFLP, and Bridge) to AltaMed HealthCare Services and initiatives. The LCC works in support of Youth Services programs which provide in-home case management support services for pregnant and parenting teens participating in the Adolescent Family Life Program (CDPH,) Cal Learn (DPSS Los Angeles County,) Bridge Program for parenting college students (AltaMed Initiative). The LCC will work as a liaison between AltaMed Youth Services clients and AltaMed Healthcare Services to ensure that that all program participants receive their desired healthcare services while navigating through this interagency collaborative, transitioning Youth Services clients to AltaMed patients. The LCC will assist Youth Services clients and their family members, ensuring that their experience when accessing AltaMed Healthcare Services is of the highest quality. This includes but is not limited to, problem solving, conflict resolution, quality of care, and enrollment assistance, scheduling of appointments and patient in-clinic appointment attendance. The LCC is responsible for relationship building during that transition of Youth Services clients becoming AltaMed Healthcare members, specifically utilizing Motivational Interviewing Skills to foster a positive environment for the participating youth. The LCC is an integral member of the care team working as a liaison between, Youth Services program participants, Youth Services Case Managers, Providers, and Medical Management to ensure timely access to specialty care. The Youth Services LCC will also conduct outreach / marketing efforts to other Cal Learn and AFLP contractors, partnering foster family agencies, and other community partners for linkage to AltaMed Healthcare Services.

Minimum Requirements

1. High School diploma required. Some college highly recommended in Social Work, Community Programing, Health Administration, Business, or other health-related field preferred.

2. Minimum of 1 (one) year of experience in social service, health related, or customer service related field required.

3. Bilingual preferred: English/Spanish

Compensation

$24.00 - $28.15 hourly
