Job Description:

The Community Inclusion Planning Coordinator oversees the Community Inclusion Planning Meeting (CIPM) program in Durham, Johnston, Wake, Cumberland, Mecklenburg, and Orange counties to promote growth of the program and ensure fidelity to the model.  

This position will allow the successful candidate to work a primarily remote schedule. Two days a week working onsite is required as well as working some evenings and weekends. This position will require extensive travel in Wake, Durham, Orange, Cumberland, Johnson, Mecklenburg, and Harnett counties for meetings and events.

Essential Functions & Duties

Provide oversight of the CIPM process for assigned areas  


  • Incumbents work together to develop and maintain CIPM related policies and procedures including all relevant documents 

  • Conduct quality assurance and fidelity measurements 

  • Ensure data is collected in a timely manner and identify CQI activities based on data trends

  • Actively promote and encourage engagement in CIPM process to providers, members and the community

Conduct CIPM meetings and develop teams to meet the needs of the individual served.


  • Facilitate CIPM meetings 

  • Ensure teams are developed and reflect the values and principles of System of Care (SOC), recovery and self-determination

  • Provide information, education and technical assistance to anyone that inquires or participates in a CIPM 

  • Develop resources that can be utilized by CIPM participants and team members

  • Ensure all CIPM team members have been adequately trained

Collaborate with community partners to address critical needs of individuals served


  • Represent the organization in community activities. 

  • Represent the organization in the community, in regard to special assistance and programs offered by the organization and increase community awareness of initiatives.

  • Consult with treatment teams, community collaborative and other groups serving MCO enrollees

  • Develop in-depth knowledge of various community systems

Collaborate with internal departments to ensure continuity of care 


  • Assist in the continuity of care for all persons referred to Care Coordination or service providers as needed

  • Provide consultation and technical assistance to all MCO departments regarding the CIPM process.  

  • Make referrals to other Alliance departments and aid in the early identification of individuals with special health care needs.

  • General knowledge of behavioral health and intellectual/developmental disabilities

  • Knowledge of all disability areas, including areas where co-occurring illnesses exist

  • Knowledge of the primary purpose and functions of community agencies

  • Knowledge of community resources

  • Excellent interpersonal skills

  • Excellent verbal and written communication skills

  • Problem solving skills

  • Conflict resolution and mediation skills

  • Ability to make prompt independent decisions based upon relevant facts

Minimum Requirements

Bachelor’s degree in a Human Services field and five (5) years of progressive experience in the field of Health Care or Human Services;

Or

Master’s degree in a Human Services field and three (3) years of experience in the field of Health Care or Human Services.

Additional Experience required:


  •      meeting/group facilitation 

  •      training and public speaking/presentation 

  •      development of plans of care/action plans 

Experience preferred:


  • policy/procedure development  

  • program development and oversight

  • lived experience

  • prefer bilingual speaker (Spanish/English)

Required license

Valid, unexpired Driver license

Salary Range

$28.12 - $35.85/ Hourly 

Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity.
