UC Health is hiring for Pharmacy Resident for UC Medical Center
Under the supervision of the Director of Pharmacy, and according to standard procedure the Pharmacy Resident (Pharmacy Practice or Specialty) is a pharmacist in training who provides comprehensive pharmaceutical care through integrated drug distribution, clinical services and teaching. The resident provides services consistent with other services provided by pharmacists at UC Health. The resident training program is jointly supported by the corporate department of UC Health Pharmacy Services.

Education: Minimum Required: PharmD. Applicants to international programs are graduates or candidates for graduation from a pharmacy degree program that is a minimum of five years in duration.
License and Certification: Residents must obtain Ohio pharmacist licensure in the first third of the program to ensure two-thirds of the residency is completed as a licensed pharmacist based on ASHP requirements. If not licensed in Ohio by the residency start date, the candidate must obtain an Ohio Intern License.
Work Experience: No experience is required.

Completing patient specific medication consults
Participates in patient care rounds focusing on drug therapy issues.
Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
Provides pharmacokinetic consultations as required, specifically addressing drugs with narrow therapeutic indices.
Detects, monitors, documents, and reports adverse drug reactions (ADRs) and medication errors (MEs).
Promotes the use of the formulary by converting non-formulary orders to formulary, when possible, but coordinates procurement of non-formulary drugs when necessary.
Counsels patients on admission or discharge medications as necessary.
Participates in the drug therapy management of medical emergencies, responds to Code Blue and Rapid Responses, and prepares medications as needed.
Facilitate discharge planning, including engaging Medication Access Services if needed, to ensure a smooth transition to alternate site of care or home.
Discusses medication order clarifications with the prescriber, documents any changes in the electronic medical record, and informs others who may be impacted of medication order changes.
Maintains competency in unique aspects of drug therapy for the patients for which they provide care, including factors related to patients of all age groups.
Indirect patient care:
Conducts one medication use evaluation and reports results to the appropriate UC Health committee.
Develop a research project plan to evaluate pharmacotherapy, implement project plan, collect data, analyze data, assess potential or future changes aimed at improving pharmacy practice, improvement of clinical care, patient safety, healthcare operations, or specific question related to patient care.
