Our Social Worker Will:
Carry out timely social work evaluations, including family dynamics, caregiver abilities, communication patterns, high-risk for suicide, neglect or abuse and plans intervention based on evaluation findings.Counsel patient and family/caregivers as needed in bereavement, stress, and other identified coping difficulties. Provide crisis intervention when necessary.Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs.Maintain records of social work services utilization and related activities for quality assurance, program development, and policies and procedures review and revision. Attend patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary group. Provide consultation, education, and support to the interdisciplinary group on psychosocial aspects of end-of-life.
