Job Title: RN Care Manager

Department: Greater Rochester Independent Practice Association (GRIPA)

Location: 100 Kings Highway South, Rochester, NY

Hours Per Week: 40 hours

Schedule: Full-time, Days


This position will be employed by the Greater Rochester Independent Practice Association, INC. (GRIPA). GRIPA was founded in 1996 as a unique collaboration between hospitals and physicians, for the purpose of simultaneously improving the quality and efficiency of health care. This position will not be employed by Rochester Regional Health (RRH).



SUMMARY

The RN Care Manager will provide direct patient support both in-person and remote. They will be responsible for orchestrating care management across the continuum, coordinating care needs with physicians and care providers, developing care plans, implementing interventions, and educating patients on chronic condition management. The RN Care Manager will act as part of an interdisciplinary care team.



RESPONSIBILITIES



  • Providing patient directed and disease specific self-management education and psychosocial support


  • Monitoring and measuring patient progress and assure timely and complete documentation following any patient related contact


  • Work in settings across the care continuum including hospitals, provider practices and patient's home and/or community settings


  • Working in collaboration with all members of the care team to assess, facilitate, plan and advocate appropriate services to meet the patient's health requirements on an individual basis


  • Facilitating quality, cost-effective medical management



REQUIRED QUALIFICATIONS



  • BSN (preferred) or Associate's Degree in Nursing (ASN) with a Bachelor's degree in another discipline


  • Five (5) years clinical experience in care management, internal medicine, acute care or home-based experience


  • Excellent communication and interpersonal skills


  • Ability to navigate complex medical and social systems


  • Exceptional documentation and planning skills


  • A collaborative and flexible spirit


  • Strong computer skills (competence in Microsoft Office, particularly Word, basic Excel, Outlook, Teams, and familiarity with electronic medical records)



PREFERRED QUALIFICATIONS:



  • Behavioral Health Experience


  • Pediatric Experience


  • Home Care Experience


  • Bilingual skills



PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.



PAY RANGE: $79,560.00 - $109,200.00

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
