Case Manager RN (Contract)Location: Remote Schedule: Monday - Friday, 8:00 AM - 4:30 PM ET Pay Rate: $38.00 - $40.00/hour
Job Description: Our client is seeking a skilled and compassionate Case Manager RN to join their team remotely. This contract role offers the opportunity to collaborate with patients, caregivers, and providers to deliver exceptional care and support.
Key Responsibilities:
Care Plan Management
Engage and collaborate with patients, caregivers, and providers to develop individualized care plans that address disease management, health promotion, and patient-centered goals.
Monitor patient progress against care plan goals, focusing on care transitions and changes in health status.
Identify barriers to achieving care plan goals and work with patients, caregivers, and interdisciplinary teams (IDT) to overcome challenges.
Patient Education & Support
Provide disease management, health promotion, and prevention education to patients, caregivers, and families to manage disease progression and encourage independence.
Document and engage in advance directive conversations, reconciling patient goals with current clinical status.
Interdisciplinary Coordination
Complete initial and ongoing patient assessments using information from multiple sources, including electronic medical records (EMR).
Manage and coordinate care services within the interdisciplinary team.
Respond to incoming clinical calls promptly, ensuring patient concerns are addressed.
Regulatory Compliance
Adhere to regulatory standards and timeframes set by the National Committee for Quality Assurance (NCQA) within Delegated Case Management markets and Dual & Chronic Special Needs Plans (SNP).
Qualifications & Requirements:
Active and unrestricted RN license in Michigan (BSN preferred).
Minimum 3+ years of clinical experience in a hospital, home care, hospice, clinic, or nursing home setting.
Experience in case management and CCM certification (preferred).
Proficiency in patient-centered care plan creation and management.
Desired experience:
1+ years of utilization management experience.
Disease state management with strong patient education skills.
Population health management experience.
Ability to manage patient caseloads using data and reports.
