This position is located at George Ricks School Based Health Center located at 757 Curtis, Schertz, TX78154

Essential Duties:


  • Applies social work theory, knowledge, methods, ethics, and the professional use of self to restore or enhance functioning, as part of generalist practice that includes interviewing, assessment, planning, intervention, evaluation, case management, information and referral, problem solving, consultation, supportive counseling, education, advocacy and community organization.

  • Work a flexible schedule which includes unscheduled visits with walk-in patients, same day patient visits, and scheduled patient visits as referred by Providers, members of the interdisciplinary team and self-referrals.

  • Develops an action plan with goals, based on functioning level, physical/medical condition, support network, psychosocial and financial status based on long and short-term needs.

  • Develops and maintains effective working relationships with individuals and/or families referred for case management services.

  • Provide social work intervention only in the context of a professional relationship.

  • A social worker shall be responsible for setting and maintaining professional boundaries.

  • Assists patients/clients who are deemed ineligible for Methodist Healthcare Ministries (MHM) services to access alternative community resources.

  • Develops and maintains a cumulative record for each client receiving case management services as required by professional licensure standards and MHM policies and procedures.


  • Works as part of an interdisciplinary team to provide integrated behavioral health and primary care services.

  • Communicates effectively with clients, MHM staff, and participants in interdisciplinary team.

  • Cooperates with and contributes to the development and implementation of an interdisciplinary treatment plan.

  • Promotes interactive communication and develops and refines linkages between MHM programs and community health care facilities, social service providers, schools, and other agencies.

  • Maintains current documentation of available community resources.

  • Conducts home visits in the community as required.

  • Adheres to established timelines for documentation and response to clients.

  • Attend and participate in meetings and Quality Assurance activities as required.

  • Implements MHM policies and procedures as established by the organization.

  • Participates in community events during the business work week, weekends and evenings as assigned or approved.

  • Sustains productivity expectations as established by the Behavioral Health Department.

  • Maintains an active LBSW as designated by the Texas Behavioral Health Executive Council, completing all required continuing education, following all statutes and rules required by the license.

  • Performs other duties as assigned.


Qualifications:

Education and/or Experience:

Bachelor's degree in Social Work required; two years of case management or related experience required.

Certificates and Licenses:

Licensed Baccalaureate Social Worker (LBSW) by the Texas State Board of Social Worker Examiners.

Language Skills:

Must be able to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Must be able to write reports, business correspondence, and procedure manuals. Must be able to effectively present information and respond to questions from groups of managers, clients, and the general public. Must speak English. Spanish speaking required.

Reasoning Ability:

Must be able to define problems, collect data, establish facts, and draw valid conclusions. Must be able to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Computer Skills:

Must be able to demonstrate working knowledge of the Internet, Microsoft Suite, Outlook, and PowerPoint, Word, Excel and electronic health records. Must be able to efficiently document in electronic health record.

Other:

Must be able to drive and have access to a car; maintain valid driver's license and auto liability insurance. Frequent in-town travel required. Periodic regional travel maybe required

While performing the duties of this job, the team member is exposed to an office and residential environment having direct contact with patients and clients. Position may necessitate bending, stooping, twisting, turning, walking, climbing step stools, sitting and standing for periods of time. Must be able to lift and maneuver at least 15 pounds. Maybe required to work evenings and/or weekends
