This allows us to take a comprehensive, holistic approach to improving the lives of every person we serve - those who receive and those who reap the benefits of our innovative, responsive solutions.GENERAL DESCRIPTION OF POSITION:Under the direct supervision of the Director of Operations, has the primary responsibility for enhancing the quality of supports to persons served by Chimes Virginia through a comprehensive person-centered planning process and approach. This position is responsible for assisting persons served and families in all phases of the admissions, review, discharge, and annual ISP processes for individuals enrolled in services through Chimes Virginia. This position serves as the coordinator for new admissions and confirms all aspects of the intake process have been completed, including securing funding. This individual will complete and ensure the quality and timeliness of assessments, accurate tracking and reporting of referral information, annual ISP meetings, and quarterly submissions into the appropriate state database. She/he may also review funding annually and/or when urgent needs arise for each individual receiving supports at Chimes.PRIMARY JOB FUNCTION(S):


  • Prepares profile information and paperwork for all new admissions, discharges, and ISP meetings.
  • Coordinates all activities related to the admissions and Individual Support Planning process to ensure compliance with all state and federal regulations, as well as internal procedures.
  • Works with the Director of Operations to identify funding needs for potential admissions and for current persons served who may require additional short or long term supports.
  • In conjunction with the Director of Operations, monitors vacancies, interviews and screens initial inquiries, mails applications, medical forms, and interviews applicants and families when appropriate regarding the admissions needs for the programs.
  • Prepares for and coordinates admissions and ISP meetings as needed or dictated by annual dates.
  • Actively works with the persons served and their support team to develop person centered outcomes and goals for the individual to achieve his/her "good life".
  • Tracks the Individual Support Planning process to ensure submission, implementation, and documentation in accordance with the Individual Support Plan.
  • Acts as the Agency liaison for local and state stakeholders, as well as family and support representatives, and other funding agencies to ensure a constant flow of referral candidates.
  • Maintains a list of services provided to all persons supported and notes changes in plans including the date of the change and service units throughout the year. Notifies the program staff and business office staff as changes occur.
  • Collaborates with internal and external stakeholders including the families, guardians, care providers, Support Coordinators, House Managers, Division Managers, Director of Operations, etc. as it relates to the Individual Support Planning process, implementation, data collection, and information updates.
  • Conducts program tours and assessments for persons served, families, and/or support coordinators.
  • Reviews initial referrals for placement and forwards all required paperwork to the appropriate department.
  • Submits reports to the Director of Operations or designated entities as directed.
  • Provides support to families and persons served during admissions and discharge process.
  • Provides advocacy services for persons served and families in order to gain access to the supports.
  • Completes data entry and uploading of documentation into appropriate systems for all new admissions, as well as annual meetings.
  • Markets the Agency in the community by attending all service provider fairs.
  • Contacts all support coordinators on a quarterly basis with a description of all residential program vacancies.
  • Works with individuals and families to provide them information about the services Chimes offers and process to get these services.
  • Uses all available technology for the completion of specified job duties.
  • Demonstrates knowledge of, and compliance to state and agency regulations and requirements. Is able to articulate these to stakeholders.
  • Contributes to a work atmosphere in a manner which is positive, enthusiastic, respectful, and courteous.
  • Obtains necessary documentation for residential and day placements in a timely manner.
  • Ensures appropriate state and agency eligibility for all prospective admissions.
  • Obtains appropriate authorization, prior to admission, from other service providers.
  • Closes persons' served file upon discharge.
  • Uses technology for the completion of specified job duties.
  • Attends work regularly according to assigned work schedule and in accordance with Agency policy.
  • Attends and participates in in-service training, staff meetings and other activities to facilitate professional development.
  • Works cooperatively with others including all staff, supervisors, administrators, co-workers, individuals served, community professionals, customers, vendors and the public.
  • Follows instructions and abides by Agency policies and procedures.
  • Is a positive role model for individuals served and Agency staff.
  • Assumes other duties, responsibilities and special projects as needed.

REQUIREMENTS:EDUCATION: Bachelor's degree from an accredited college or university with a concentration in psychology, social work, human services, or related field preferred.EXPERIENCE: Two year of admissions experience with an Agency serving people with intellectual disabilities. Less experience may be accepted with higher level of Education completed. Must have an acceptable driving record as determined by criteria established by the Agency's insurance carrier and by Agency policy. NOTE: At the discretion of the Personnel Officer, additional related experience and/or education may be substituted in lieu of the requirements specified above under education and experience.What's in it for you?
