Location



On-site in our Haverhill, MA location 4 days a week and 1 day remote. Initial training will be located in a combination of our pediatric practice in Andover and routine (bi-monthly) meetings at our corporate headquarters in Assembly Row- Somerville, MA.



Schedule



Salaried Full-time, 40 hours per week - Monday through Friday from 8:30am to 5:00pm/ 8:00am to 4:30pm

The Pediatric Clinical SW Care Manager is a hybrid social worker role. It will consist of dual job responsibilities. First part of the role is to work with pediatricians in practices to care for patients with urgent mental health/behavioral health needs. The second part of the role is providing care management to a panel of complex pediatric patients with longer term mental/behavioral health needs.



Primary Responsibilities



  • Reviews and assists in triaging new iCMP patients with the PCP, RN Care Manager and other members of the iCMP care team, as appropriate.

  • Provides comprehensive assessment of patients and families to evaluate clinical and supports needed.

  • Identifies key barriers for a successful outcome.

  • Monitors the patient's progress and comprehensive care plan. Intervenes to ensure the plan of care and services provided are patient focused, high quality, efficient and cost effective.

  • Monitors patients in acute and non-acute facilities, assesses and identifies patients with complex medical or mental health discharge planning and continuing care issues and refers to other members of the care team, including nurse care manager as appropriate

  • Coordinates and attends family/team meetings, school meetings and community meetings when appropriate.

  • Presents case reviews/rounds.






Qualifications

Qualifications:



  • LICSW or LMHC

  • At least 3 years of experience with majority of pediatric population. Ideally with experience in private/public educational systems, trauma and/or community mental and behavioral health centering on holistic patient and family care.

  • Clinical experience with urgent/crisis situations and high-risk social situations


Preferred:



  • Bilingual skills preferred
