Location
On-site in our Haverhill, MA location 4 days a week and 1 day remote. Initial training will be located in a combination of our pediatric practice in Andover and routine (bi-monthly) meetings at our corporate headquarters in Assembly Row- Somerville, MA.
Schedule
Salaried Full-time, 40 hours per week - Monday through Friday from 8:30am to 5:00pm/ 8:00am to 4:30pm

The Pediatric Clinical SW Care Manager is a hybrid social worker role. It will consist of dual job responsibilities. First part of the role is to work with pediatricians in practices to care for patients with urgent mental health/behavioral health needs. The second part of the role is providing care management to a panel of complex pediatric patients with longer term mental/behavioral health needs.
