SUMMARY

Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS).  HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County.  Our organization follows a hybrid work structure where employees work both remotely and from the office, as needed.  

The Assistant Staff Analyst (ASA), Facility Improvement position will report to the Director of Enriched Residential Care Program (ERC) who manages the daily operations of the Enriched Residential Care program which provides permanent housing for homeless individuals who need assistance with their activities of daily living in a 24-hour licensed care facility.

The Assistant Staff Analyst (ASA) is needed to assist in overseeing and managing a portfolio of RCFEs and ARFs in one of three designated regions in Los Angeles County that serve for homeless individuals, seniors and adults with disabilities who require care and supervision to be stably housed.  It is expected that the ASA will ensure the quality of services and accommodations for ERC clients in RCFE facilities by conducting quarterly on-site evaluation, developing continuous quality improvement plans for each facility, providing technical assistance and training and implementing accountability measures when necessary.

ESSENTIAL FUNCTIONS


  • Conduct quarterly facility visits to a portfolio of ERC facilities within an assigned region to assess the safety and quality of facility services.

  • Conduct an assessment and grading of facilities utilizing agreed upon tool.

  • Develop an improvement plan that identifies areas of concern and applicable resources, required trainings and/or participation in technical assistance.

  • Provide onsite technical assistance in collaboration with the Referral and Placement Team (RAP) and the client wellness team.

  • Identify new facilities and remove facilities that are not improving or otherwise create concern regarding the immediate well-being of ERC clients.

  • Communicate with the placement team (RAP) of any changes to ERC facilities.

  • Communicate with CCLD to identify facilities that are of immediate concern to the safety and well-being of clients.

  • Communicate with Los Angeles Department of Mental Health (DMH), The Office of Diversion and Reentry (ODR) and Brilliant Corners and identify facilities that are of immediate concern to the safety and well-being of clients.

  • Maintain current and up to date master facility list in CHAMP.

  • Convene quarterly learning communities with facility operators.

  • Coordinate monthly internal team meetings to discuss facilities challenges and training opportunities and identify high performing facilities.

  • Oversee all ERC contracts and agreements and work with the data team to finalize reporting to funders and internal stakeholders.

  • Maintain oversight of all ERC facility agreements in collaboration with Brilliant Corners.

  • Coordinate annual monitoring visits with the PSH team and oversee facility corrective action plans.

  • Identify themes that emerge for training and technical assistance for facility administrators.

  • Participate in quarterly technical assistance calls with facility operators.

  • Participate in quarterly learning communities for facility operators.

  • Participate in monthly team meetings.

JOB QUALIFICATIONS

Three years of experience in a staff capacity analyzing and making recommendations for the solution of problems of organization, program, procedure, budget or personnel -OR- Two years of highly responsible administrative or staff experience -OR- One year of responsible staff experience in health care or community or social service operations, -OR- Working in a program that provides housing and support services to clients who are homeless or are at risk for homelessness; -OR- Making recommendations for the design and/or implementation or evaluation of programs related to health and human services for high-risk populations (e.g. homeless, justice-involved, mentally ill, substance use disorder or medically complex).

Certificates/Licenses/Clearances


  • A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

  • Successful clearing through the Live Scan and the Health Clearance process with the County of Los Angeles.

Other Skills, Knowledge, and Abilities


  • Homeless health care experience.

  • Experience with delivering clinical care services within ARFs and RCFEs.

  • Familiarity with complex care management services for the care of high-need/high-cost MediCal beneficiaries.

  • Ability to lead teams in performance improvement activities.

  • Demonstrated teaching and communication skills with non-clinical and clinical staff.

  • Experience/expertise in assessing and managing clients with dysfunctional substance use disorder (SUD), including knowledge of MAT (Medication-Assisted Treatment) and Addiction Psychiatry.

  • Experience/expertise in assessing and managing clients with serious persistent mental illness.

  • Experience/expertise in trauma-informed care and harm reduction principles.

  • Familiarity with DMH, DPH and community mental health and substance use programs and resources.
